Endovascular versus open elephant trunk completion for extensive aortic disease
Objectives: To compare the outcomes between patients undergoing endovascular (EEC) or open (OEC) approaches to second-stage elephant trunk completion (EC).Methods: From 1993 to 2010, 225 patients underwent second-stage EC (EEC, n = 92; OEC, n = 133). Propensity matching was performed for a fair comparison.Results: The EEC patients were older, more likely to have atrial fibrillation, and had a smaller proximal aorta. Repair of retrograde ascending dissection after descending stent grafting
Conclusions: Retrograde ascending dissection can present as an early or a late complication after descending stent grafting because of aortic instability or disease progression and has usually been associated with descending dissection or intramural hematoma. It is a life-threatening complication that can be managed safely with early recognition and rapid delivery of open or hybrid repair.

Conclusion: Emergency surgery for ATAAD in elderly patients can be performed with acceptable early mortality and satisfactory intermediate survival. Preoperative acute neurological deficit predicts a worse outcome. Advanced age alone is not a contraindication to ATAAD repair.

Aggressive intraoperative debridement with in situ prosthetic reconstruction permits a high rate of success in this very high risk cohort of patients. Lifelong antibiotic suppression therapy may prevent late prosthetic graft infection.

Central cannulation strategy for extent I thoracoabdominal aneurysm repair of chronic type B aortic dissection
ConclusionsCentral true lumen cannulation through a left thoracotomy incision for repair of extent I TAAA with chronic type B dissection is an acceptable approach with equivalent early and midterm outcomes compared to more standard femoral cannulation techniques. It may provide a safe alternative cannulation site for patients with diseased femoral vessels.
