CONCLUSION: Cognitive decline significantly elevates the risk of diabetic vascular complications and is correlated with retinal and renal microcirculation damage. Pole to pole
An otherwise asymptomatic 67-year-old man presented to his ophthalmologist complaining of acute painless “dark area on the right”. Visual acuity was preserved and a single cotton-wool spot was noted in each retina. An inferior right quadrantanopia was evident on automated visual field and computerized tomography of the brain confirmed a left occipital stroke. Acute phase markers were elevated and t emporal artery biopsy was consistent with a diagnosis of giant cell arteritis. Isolated retinal cotton wool spots, even in the absence of systemic signs and symptoms, may be suggestive of giant cell arteritis.

Rational clinical approach to transient ischaemic attack, stroke and associated mimics: A retrospective cohort study
CONCLUSIONS: The present study demonstrated that specific clinical features and risk factors were associated with the final diagnosis at TIA clinic. Adult-onset leukodystrophy with vanishing white matter: a case series of 19 patients
ConclusionsAO-LVWM presents varying clinical manifestations at onset, including stroke-like events. WM rarefaction is the most consistent diagnostic clue even in the latest onset cases. Spectroscopy and electrophysiological features are compatible with axon, rather than myelin, damage. Cerebral glucose metabolic abnormalities and retinal alterations can be present. LVWM might also be caused by a digenic inheritance affecting the eIF2B complex.
