Large-Scale Phenome-Wide Association Study of PCSK9 Variants Demonstrates Protection Against Ischemic Stroke Original Articles
Conclusions: This result represents the first genetic evidence in a large cohort for the protective effect of PCSK9 inhibition on ischemic stroke and corroborates exploratory evidence from clinical trials. PCSK9 inhibition was not associated with variables other than those related to LDL (low-density lipoprotein) cholesterol, atherosclerosis, and type 2 diabetes mellitus, suggesting that other effects are either small or absent.

Re: Prognosis of Neurological Improvement in Inpatient Acute Ischemic Stroke Survivors: A Propensity Score Matching Snalysis
Wu Q and colleagues1 conducted a propensity-score (PS) matched study to identify prediction factors associated with short-term outcomes in patients with acute ischemic stroke survivors (improvement/ no improvement). In an effort to control for confounding, the authors matched on the propensity score to balance the National Institute of Health Stroke Scale (NIHSS) at admission between the two outcome groups. After matching, 92 pairs were analyzed in a multivariable outcome model of which two variables, diabetes and cognitive assessment, were identified as independent risk factors for the prognosis of neurological improvement.

Cardiovascular Risk Profile and Cognitive Function in Young, Middle-Aged, and Elderly Subjects Clinical Sciences
Conclusions— In this large population–based cohort, a worse overall cardiovascular risk profile was associated with poorer cognitive function. This association was already present in young adults aged 35 to 44 years.

Duration of Diabetes Mellitus and Risk of Thromboembolism and Bleeding in Atrial Fibrillation: Nationwide Cohort Study Clinical Sciences
Conclusions— In patients with atrial fibrillation, longer duration of diabetes mellitus was associated with a higher risk of thromboembolism, but not with a higher risk of anticoagulant-related bleeding. Considering the critical balance between preventing thromboembolism and avoiding bleeding, longer duration of diabetes mellitus may favor initiation of anticoagulant therapy.

Conclusions— AII are more likely to be present in those with cognitive impairment. Although a cause–effect relationship between the presence of AII and cognitive impairment is plausible, the association may be because of under-reporting of symptoms by individuals with cognitive impairment. The association between AII and cerebral microbleeds may indicate cerebral vasculopathy, independent of traditional vascular risk factors.
