Potential new uses of non-vitamin K antagonist oral anticoagulants to treat and prevent stroke
Conclusion: There may be a role for NOACs in stroke prevention and treatment beyond atrial fibrillation. Randomized controlled trials are needed to compare NOACs to current stroke prevention and treatment strategies in certain subgroups of patients with cerebrovascular disease.

Association of Hemoglobin A1c and Baseline Ischemic Stroke Severity: A Population-Based Study (P1.187)
Conclusions: We found that elevated HbA1c was associated with decreased stroke severity at presentation. However, this effect is mediated by the history of DM rather than a specific dose-effect of glycemic control. Our results provide evidence that although chronic hyperglycemia increases risk of stroke, it is not associated with increased stroke severity.Disclosure: Dr. Yang has nothing to disclose. Practice advisory: Recurrent stroke with patent foramen ovale (update of practice parameter): Report of the Guideline Development, Dissemination, and Implementation Subcommittee of the American Academy of Neurology
Conclusions: Percutaneous PFO closure with the STARFlex device possibly does not provide a benefit in preventing stroke vs medical therapy alone (risk difference [RD] 0.13%, 95% confidence interval [CI] –2.2% to 2.0%). Percutaneous PFO closure with the AMPLATZER PFO Occluder possibly decreases the risk of recurrent stroke (RD –1.68%, 95% CI –3.18% to –0.19%), possibly increases the risk of new-onset atrial fibrillation (AF) (RD 1.64%, 95% CI 0.07%–3.2%), and is highly likely to be associated with a procedural complication risk of 3.4% (95% CI 2.3%–5%). There is insufficient evidence to d...

Circulating biomarkers and incident ischemic stroke in the Framingham Offspring Study
Conclusions: Higher levels of 4 biomarkers—CRP, tHcy, TNFR2, and VEGF—increased risk of IIS and improved the predictive ability of the Framingham Stroke Risk Profile score. Further research is warranted to explore their role as potential therapeutic targets.

Can admission Brain Natriuretic Peptide level predict outcome after intravenous thrombolysis in acute ischemic stroke? (P4.287)
Conclusions:Elevated BNP level was not found to be an independent marker of poor outcome in acute ischemic stroke patients following intravenous thrombolysis.Disclosure: Dr. Gupta has nothing to disclose. Predictors of atrial fibrillation in patients with cryptogenic stroke (P6.294)
Conclusions:Consistent with prior studies, atrial fibrillation was found by outpatient monitoring in a notable percentage of cryptogenic stroke patients. Predictors of occult atrial fibrillation in our study population included PACs on EKG and higher TSH levels. Although an association between hyperthyroidism and atrial fibrillation has been well established, our results intriguingly suggest that hypothyroidism may be a predictive factor as well.Disclosure: Dr. Renati has nothing to disclose. Effectiveness and Safety of Apixaban and Rivaroxaban Versus Warfarin for the Secondary Prevention of Stroke or Systemic Embolism Among Nonvalvular Atrial Fibrillation Patients (S51.001)
Conclusions:Among NVAF patients with a prior history of SSE, rivaroxaban reduced ischemic stroke and ICH risk versus warfarin and did not significantly impact major bleeding. Apixaban did not significantly alter patients’ risk of SSE or major bleeding compared to warfarin in this high-risk population.Study Supported by: Bayer Pharma AGDisclosure: Dr. Coleman has received personal compensation for activities with Janssen Scientific affairs, LLC, Bayer Pharma AG, and BIPI . Age and the fuzzy edges of embolic stroke of undetermined source: Implications for trials
After an acute ischemic stroke, clinicians pursue further diagnostic assessment to identify the most plausible causes and tailor secondary prevention strategies. Available treatments with a high level of evidence for reducing recurrence stroke risk might include long-term oral anticoagulation in patients with a major-risk cardioembolic source (atrial fibrillation, prosthetic valves, etc), carotid endarterectomy in minor ischemic stroke related to moderate to severe internal carotid artery atherosclerotic stenosis, and best-practice medical management in those with lacunar stroke due to cerebral small vessel disease.1 Howev...

The Case for Selective Patent Foramen Ovale Closure After Cryptogenic Stroke Advances in Interventional Cardiology
Controversy has persisted for over a decade whether transcatheter patent foramen ovale (PFO) closure reduces the rate of recurrent ischemic stroke for patients who have had a cryptogenic ischemic stroke and have a PFO. In September 2017, 3 positive randomized trials: RESPECT long-term (Recurrent Stroke Comparing PFO Closure to Established Current Standard of Care Treatment trial), REDUCE (GORE® HELEX® Septal Occluder / GORE® CARDIOFORM Septal Occluder and Antiplatelet Medical Management for Reduction of Recurrent Stroke or Imaging-Confirmed TIA in Patients With Patent Foramen Ovale [PFO]), and CLOSE (Patent For...

Determinants of Early Outcomes in Patients with Acute Ischemic Stroke and Proximal Artery Occlusion (S02.005)
CONCLUSIONS: In acute ischemic stroke patients thought LTB from IAT, IV tPA independently predicted favorable early post-stroke outcomes. These data reinforce the recommendation to provide early, rapid IV tPA to all eligible patients. Further studies are warranted to explore the lack of IAT effect seen in this study, which may be due to small sample size, residual confounding, or interaction between the IV tPA and IAT.Disclosure: Dr. Labuzetta has nothing to disclose. Incidence of First Stroke: A Population Study in Iceland Brief Reports
Conclusions— Incidence of first stroke in Iceland is similar to other Western countries. The high number of paroxysmal atrial fibrillation found during the 24-hour ECG suggests that atrial fibrillation may be underdiagnosed in patients with stroke.

Impact of atrial fibrillation on stroke risk eliminated with multiple risk factors
Dr Benn Christiansen said: "We know that atrial fibrillation increases the risk of ischemic stroke. And in patients with atrial fibrillation or previous ischemic stroke, the risk of stroke increases with the number of risk factors. But until now, little attention has been paid to the association between stroke risk and risk factors in patients without prior stroke or atrial fibrillation. We wanted to explore that association and to quantify if stroke risk was of comparable size in patients with numerous risk factors...

Optimal Timing and Duration of Continuous Electrocardiographic Monitoring for Detecting Atrial Fibrillation in Stroke Patients
Conclusions: We suggest that in order to enhance the detection rate of AF, CEM could be generalized in the stroke unit. It must be started early in patients with acute stroke and prolonged over a minimum of 4 days.

The Impact Factors on the Cost and Length of Stay among Acute Ischemic Stroke
Conclusions: Age 65 and over, atrial fibrillation, stroke treatment, and subtypes were the significant predictive factors affecting hospital costs and LOS. Compared to other countries, Taiwan spent the least while Canada had the highest expense. The United States had the shortest LOS (6 days) in contrast to Canada with the longest LOS (34-47 days). Regarding mean daily cost of stroke, the United States had the highest cost per day while China spent the least.
