Racial Differences in Triage for Emergency Department Patients with Subjective Chief Complaints
In this study we aimed to determine whether these effects are more pronounced for patients with subjective complaints. Methods: We performed a retrospective analysis for all adult visits between 2016-2019 at an urban academic emergency department (ED) with acuity-based pods. We determined rates of initial high-acuity triage both across all patients and among the subset located in the high-acuity pod at time of disposition (either through initial assignment or subsequent up-triage). Analysis was performed for common chief complaints categorized as subjective (chest pain, dyspnea, any pain); observed (altered mental status); ...

Neurologic emergencies can be difficult to diagnose and treat, and are devastating if missed. Stroke is still the number five cause of death in the United States, down from number three a few years ago, and is the leading cause of long-term serious disability. Nonconvulsive status epilepticus is a substantial problem in intensive care units. Traumatic brain injuries from head trauma are a leading cause of post-traumatic stress in many populations. Then we have spine injuries, headache, back pain, infections, and vision problems, which present to emergency departments on a daily basis.

Stroke-like manifestation of a spontaneous spinal epidural hematoma with spontaneous resolution: a case report
ConclusionsSEH, although uncommon, can be a mimic of stroke and it is important to avoid misdiagnosis as it is a time critical diagnosis, and administration of thrombolysis or antiplatelets can lead to unfavourable outcomes. Having a high clinical suspicion can help to guide us in the choice of imaging and interpretation of subtle signs to reach the correct diagnosis in a timely manner. Further research is required to better understand the factors that would favour a conservative approach as opposed to surgical treatment.

Patterns and predictors of emergency medical services utilisation by patients attending the emergency medicine department of a tertiary care hospital in India
Conclusion: There was underutilization of the EMS services. Majority of the ambulances were not adequately equipped/staffed to deliver prehospital interventions. Policies at national level are required to encourage EMS utilization by the public and urgent measures are needed to improve services provided by them.

Characteristics and Outcomes of Pediatric Patients With a Ventricular Assist Device Presenting to the Emergency Department
Conclusions Among subjects in a single outpatient pediatric VAD program presenting to the ED, the most common complaints were abdominal pain/vomiting, fever, and headache. The most common adverse events were major infection and right heart failure. Subjects had a high rate of ED utilization and hospital admission.

Sudden Death in High School Athletes: A Case Series Examining the Influence of Sickle Cell Trait
We present a case series of fatal ECAST in high school athletes aged 14 to 16 years. All 3 athletes experienced collapse during practice sessions with muscle pain or weakness. Upon evaluation at the hospital, the athletes had a significant metabolic acidosis that did not respond as expected to fluid resuscitation. Admitting diagnoses for the athletes included exertional heat stroke or dehydration. All 3 athletes had profound rhabdomyolysis leading to acute renal failure, worsening metabolic acidosis, and hyperkalemia. They rapidly progressed to disseminated intravascular coagulation, multiorgan system failure, and death. T...

Recognizing Vaccine-Induced Immune Thrombotic Thrombocytopenia
CONCLUSIONS: Vaccine-induced immune thrombotic thrombocytopenia is a serious complication of vaccination that is not feasible to anticipate or prevent. When the patient presents with sustained headache, neurologic symptoms/signs, abdominal pain, dyspnea, or limb pain/swelling beginning 5–30 days post vaccination, platelet count and d-dimer must be measured, and imaging for thrombosis performed. Confirmation of vaccine-induced immune thrombotic thrombocytopenia diagnosis should be ordered (platelet factor 4/polyanion enzyme-linked immunosorbent assay; platelet factor 4–enhanced platelet activation testing) as treatm...

European Resuscitation Council Guidelines 2021: First aid
The European Resuscitation Council has produced these first aid guidelines, which are based on the 2020 International Consensus on Cardiopulmonary Resuscitation Science with Treatment Recommendations. The topics include the first aid management of emergency medicine and trauma. For medical emergencies the following content is covered: recovery position, optimal positioning for shock, bronchodilator administration for asthma, recognition of stroke, early aspirin for chest pain, second dose of adrenaline for anaphylaxis, management of hypoglycaemia, oral rehydration solutions for treating exertion-related dehydration, manage...

Marion County (WV) Rescue Squad Honored for Heart Patient Transport Times
Eddie Trizzino Times West Virginian, Fairmont (MCT) When an individual is having a heart attack, every second matters in getting them treated to ensure they will not only survive, but maintain a healthy heart after their trauma. For a few months this past summer, the Marion County Rescue Squad could not transfer patients to an emergency department within Marion County, after Fairmont Regional Medical Center closed in May. Michael Angelucci, administrator of the Marion County Rescue Squad, in some cases, this added up to 30 minutes to a patient’s transport to a hospital. ...
