Novel approaches to hypoglycemia and burnt-out diabetes in chronic kidney disease
Purpose of review Diabetes mellitus is a leading cause of chronic kidney disease (CKD) that confers faster kidney disease progression, higher mortality, and various metabolic derangements including hypoglycemia. Recent findings Even in the absence of diabetes mellitus, growing research demonstrates that CKD patients are at heightened risk for hypoglycemia via multiple pathways. In CKD patients transitioning to end-stage renal disease (ESRD), spontaneous resolution of hyperglycemia and frequent hypoglycemia resulting in reduction and/or cessation of glucose-lowering medications are frequently observed in a phenom...

The Relationship Between Atrial Fibrillation, Mitral Regurgitation, and Heart Failure Subtype: The ARIC Study
CONCLUSIONS: In patients with ADHF, AF prevalence increased with MR severity and this effect was more pronounced in HFpEF compared with HFrEF. AF was associated with an increased 1-year mortality only in patients with HFpEF and concomitant moderate/severe MR.REGISTRATION: NCT00005131, https://clinicaltrials.gov/ct2/show/NCT00005131.PMID:34955335 | DOI:10.1016/j.cardfail.2021.10.015

Association of peridialytic, intradialytic, scheduled interdialytic and ambulatory BP recordings with cardiovascular events in hemodialysis patients
CONCLUSIONS: Averaged intradialytic and scheduled home BP measurements (but not pre-dialysis readings) display similar prognostic associations with 44-h ambulatory BP in hemodialysis patients and represent valid metrics for hypertension management in these individuals.PMID:34988941 | DOI:10.1007/s40620-021-01205-9

Impact of atrial fibrillation and the clinical outcomes in patients with acute myocardial infarction from the K-ACTIVE registry
CONCLUSION: In Japanese AMI patients, AF was often observed and was associated with worse MACE but not worse bleeding.PMID:35216889 | DOI:10.1016/j.jjcc.2022.02.007

Impact of left ventricular hypertrophy on clinical outcomes in patients with dialysis: a single-center study in Japan
ConclusionJapanese patients with LVH undergoing dialysis had a worse prognosis than those without LVH in terms of the composite clinical endpoint.

Extent of mural thrombus is not associated with increased 5-year mortality following elective AAA repair
CONCLUSIONS: In our experience, the extent of mural thrombus in AAA does not influence long-term survival after elective repair. AAA repair may provide protection against circulating components of mural thrombus which have the potential to promote atherosclerotic-related adverse events. Patients with renal insufficiency and larger AAA have increased risk of mortality 5 years after elective repair.PMID:35331063 | DOI:10.1177/17085381211063282

Hypoxia-inducible factor stabilisers for the anaemia of chronic kidney disease
CONCLUSIONS: HIF stabiliser management of anaemia had uncertain effects on CV death, fatigue, death (any cause), CV outcomes, and kidney failure compared to placebo or ESAs. Compared to placebo or ESAs, HIF stabiliser management of anaemia probably decreased the proportion of patients requiring blood transfusions, and probably increased the proportion of patients reaching the target Hb when compared to placebo.PMID:36005278 | DOI:10.1002/14651858.CD013751.pub2

Valacyclovir-associated acute kidney injury and encephalopathy in an elderly woman with normal kidney function: a case report
AbstractA 72-year-old Japanese woman was treated by 3000  mg/day of valacyclovir for the herpes zoster in her left back. She had been treated as hypertension with no renal insufficiency. In two days, she visited an emergency room of a regional stroke care center with dysarthria, dexterity disorder and gait disturbance. Neither head CT nor MRI found intra cranial lesions, then, laboratory tests revealed that her serum creatinine level was 4.63 mg/dL. She was transferred and admitted to our hospital on the following day and received hemodialysis under the diagnosis of AKI due to acyclovir accompanied with encephalopathy. A...

Dietary evaluation of sodium intake in patients with chronic kidney disease
CONCLUSION: the non-correlation between sodium obtained by 24-hour urinary excretion and dietary intake demonstrates the fragility of the estimation of sodium excretion through the dietary survey.PMID:36537325 | DOI:10.20960/nh.04255
