Ascending-to-Descending Aoritc Bypass: A Simple Solution to a Complex Problem.
CONCLUSIONS: The ascending-to-descending aortic bypass can be performed with low morbidity and mortality. It is an effective solution to complex aortic coarctation and represents a safe single-stage approach for patients with concomitant cardiac pathology. Short-Term Results of Transapical Transcatheter Mitral Valve Implantation for Mitral Regurgitation
ConclusionsTransapical transcatheter mitral valve implantation is technically feasible and can be performed safely. Early hemodynamic performance of the prosthesis was excellent. Transcatheter mitral valve implantation may become an important treatment option for patients with severe MR who are at high operative risk.

Double-Barrel Proximal Thoracic Aorta or Horrific Complication of Endocarditis?
A 76-YEAR-OLD man with hypertension, hyperlipidemia, and a recent embolic stroke was admitted to the authors’ institution for evaluation of progressive dyspnea. The patient’s past medical history was notable for an aortic valve replacement with a mechanical prosthesis. He did well for approximately 30 years after surgery before he presented to a community hospital with a non-ST segment elevation myocardial infarction that was treated with metoprolol and a bare metal stent (first obtuse marginal branch of the left circumflex coronary artery).

Pontine infarction responsible for wall-eyed bilateral internuclear ophthalmoplegia syndrome
A 68-year-old man with hypertension, cardiac arrhythmia, and a mechanical heart valve replacement treated with vitamin K antagonists was admitted for the acute onset of diplopia. Neuro-ophthalmologic examination showed a bilateral exotropia in primary gaze position (wall-eyed) with bilateral adduction deficit that did not improve by convergence (figure, video at Neurology.org/cp). Postoperative Atrial Fibrillation After Thoracic Aortic Surgery
Conclusions Several risk factors contribute to the incidence of POAF after thoracic aortic surgery. We found that POAF significantly increased 30-day operative mortality (p &lt; 0.0001). Our findings can be used to develop a risk stratification system for the prediction of POAF.

Recurrent ischemic strokes and headaches originating from Lambl's excrescence (P2.273)
CONCLUSIONS: The association between recurrent strokes and late-age onset migraine with aura should raise clinical suspicion of a cardiogenic valvular etiology. Early TEE use is necessary to diagnose cardiac valvular strands like LE. LE associated with migraine headaches should prompt a treatment with anti-thrombotic agent. While anticoagulants and cardiothoracic surgery are indicated for recurrent strokes, a combined antiplatelet therapy was effective in our patient.Disclosure: Dr. Dumitrascu has nothing to disclose. Characteristics, complications, and gaps in evidence-based interventions in rheumatic heart disease: the Global Rheumatic Heart Disease Registry (the REMEDY study)
Conclusion Rheumatic heart disease patients were young, predominantly female, and had high prevalence of major cardiovascular complications. There is suboptimal utilization of secondary antibiotic prophylaxis, oral anti-coagulation, and contraception, and variations in the use of percutaneous and surgical interventions by country income level.
