Transcatheter aortic valve replacement in aortic stenosis and cardiac amyloidosis: a systematic review and meta ‐analysis
ConclusionsTAVR is an effective and safe procedure in CA-AS patients, with a substantial survival benefit as compared with medical therapy, and a safety profile comparable with patients with AS alone except for a trend towards higher risk of permanent pacemaker implantation.

Peak O2 ‐pulse predicts exercise training‐induced changes in peak V̇O2 in heart failure with preserved ejection fraction
ConclusionsIn patients with HFpEF, the easily measurable peak O2-pulse seems to be a good indicator of the potential for improving peak V ̇O2 through exercise training. While changes in submaximal exercise tolerance were independent of baseline peak O2-pulse, patients with high O2-pulse may need to use additional therapies to significantly increase peak V ̇O2.

Effect of β‐blocker on patients with moderate functional mitral regurgitation undergoing surgical aortic valve replacement
ConclusionsThe administration of BB after SAVR was not associated with lower risk of MACCE for patients of severe aortic valve disease complicated with moderate FMR, but was potentially beneficial for improving FMR.

Oral milrinone for management of refractory right ventricular failure in patients with left ventricular assist devices
ConclusionsOral milrinone appears safe for long-term use in the outpatient setting when combined with therapeutic monitoring in this complex medical cohort with limited management options. Further study is needed to ascertain whether this treatment is effective in reducing heart failure symptoms and admissions.

Characteristics and long ‐term survival of patients with left ventricular non‐compaction cardiomyopathy
ConclusionsOur study questions the criteria being used for the diagnosis of LVNC. Further evaluation of CMR variables and association of these findings with demographic variables and survival is mandatory.

Adaptive versus maladaptive right ventricular remodelling
AbstractRight ventricular (RV) function and its adaptation to increased afterload [RV –pulmonary arterial (PA) coupling] are crucial in various types of pulmonary hypertension, determining symptomatology and outcome. In the course of disease progression and increasing afterload, the right ventricle undergoes adaptive remodelling to maintain right-sided cardiac output by increasing contractility. Exhaustion of compensatory RV remodelling (RV–PA uncoupling) finally leads to maladaptation and increase of cardiac volumes, resulting in heart failure. The gold-standard measurement of RV–PA coupling is the ratio of contract...

Heart failure with reduced ejection fraction and atrial fibrillation: a Sub ‐Saharan African perspective
AbstractCardiovascular diseases are a well-established cause of death in high-income countries. In the last 20  years, Sub-Saharan Africa (SSA) has seen one of the sharpest increases in cardiovascular disease-related mortality, superseding that of infectious diseases, including HIV/AIDS, in South Africa. This increase is evidenced by a growing burden of heart failure and atrial fibrillation (AF) risk factor s. AF is a common comorbidity of heart failure with reduced ejection fraction (HFrEF), which predisposes to an increased risk of stroke, rehospitalizations, and mortality compared with patients in sinus rhythm. AF had ...

Factors influencing left ventricular thrombus resolution and its significance on clinical outcomes
ConclusionsThis study suggests that LVT resolution is an important predictor for favourable clinical outcomes. LVEF improvement failure interfered with LVT resolution and appeared to be a crucial factor for LVT recurrence. After LVT resolution, continuation of anticoagulation did not seem to impact LVT recurrence and the prognosis.

Adrenomedullin peptides and precursor levels in relation to haemodynamics and prognosis after heart transplantation
ConclusionsElevated plasma levels of ADM may be a marker of pressure/volume overload in HF patients with PH, as well as long-term prognosis after HT. In line with previous studies, our findings additionally confirm that ADM may be a marker of venous congestion in HF. Further studies are encouraged to establish a deeper understanding of the properties of ADM and its relationship with HF and PH, in order to potentially facilitate clinical management of HF and associated PH.

A case report of a patient with heart failure with preserved ejection fraction presented as dysphagia
AbstractDysphagia is a common clinical symptom in older people that can be attributed to a wide range of diseases, extending from neoplasm to gastroesophageal reflux diseases such as stroke or achalasia. We are presenting a case of a 78-year-old male with a history of heart failure with preserved ejection fraction and progressive dysphagia, due to a rare case, namely, dysphagia megalatriensis. Even though left ventricular ejection fraction was preserved, the patient improved, when we provided him with optimal medical heart failure with reduced ejection fraction treatment. In our case report, we intend to highlight the bene...

Strategic multimodal non ‐invasive assessment of cardiac performance in patients with heart failure
ConclusionsThe combination of NT-proBNP and Echo-LVFP could be used to predict adverse outcomes in patients with HF of various stages. Left atrial mechanics and exercise capacity are incremental to prognostication. Non-invasive test findings could be strategically combined to provide an integrative profile of cardiac performance.

Improved mortality and haemodynamics with milrinone in cardiogenic shock due to acute decompensated heart failure
ConclusionsThe use of milrinone compared with dobutamine in patients with ADHF-CS is associated with lower 30  day mortality and improved haemodynamics. These findings warrant further study in future randomized controlled trials.
