Variations and characteristics of the various clinical phenotypes in a cohort of Nigerian sickle cell patients
CONCLUSION: The prevalence of SLU, AVN, priapism, nephropathy and the other complications of SCD show some variations from other studies. This variation in the clinical parameters across different clinical phenotypes indicates an interplay between age, genetic and environmental factors.PMID:34493173 | DOI:10.1080/16078454.2021.1972242

Conventional and molecular imaging in sickle cell disease
Conclusions: Central nervous system: About 25% of SCD patients are affected by vascular involvement of central nervous system. CT scan has been used to detect cerebral infarction. Subclinical cranial MRI lesions yield important predictive values as a risk factor of stroke, even when detected in asymptomatic patients. SPECT is a useful technique for early detection of deficits in cerebral perfusion in patients with SCD. Decreases in cerebral perfusions may lead to silent infarction, stroke, or neurocognitive diseases. PET may improve sensitivity in detection of impaired metabolism in the area surrounding a major vessel infa...

Hypertrophic Pachymeningitis Related Brain Infarction in Synovitis-Acne-Pustulosis-Hyperostosis Osteomyelitis Syndrome
A 50-year-old woman with a history of synovitis-acne-pustulosis-hyperostosis osteomyelitis (SAPHO) syndrome was admitted for left unilateral neglect, dysarthria, and left hemiparesis. Brain MRI showed multiple infarctions in the territory of the right middle cerebral artery and gadolinium enhancement of the thickened frontotemporal dura mater on the right side. MR angiography showed significant narrowing of the cavernous segment of the right internal carotid artery. The right internal carotid artery stenosis was thought to originate from hypertrophic pachymeningitis associated with SAPHO syndrome.

Co-Inheritance of alpha-thalassemia and sickle cell disease in a cohort of Angolan pediatric patients.
Abstract The aim of this study was to explore the association between alpha-thalassemia, fetal hemoglobin, hematological indices, and clinical adverse events in Angolan sickle cell disease pediatric patients. A total of 200 sickle cell disease (SCD) children were sampled in Luanda and Caxito. A venous blood sample was collected and used for hematological analyses, fetal hemoglobin quantification, and genotyping of 3.7 kb alpha-thalassemia deletion by GAP-PCR. The frequency of the 3.7 kb alpha-thalassemia deletion in homozygosity was 12.5% and in heterozygosity was 55.0%. An increase in alpha-thalassemia frequency ...

F-18 FDG PET/CT in Management of Necrotizing Otitis Externa
1157Objectives: Necrotizing otitis externa (NOE), also referred to as "malignant otitis externa," is an uncommon complication of external ear canal infections, resulting in skull base osteomyelitis. Multiple pathogens have been implicated, the most common of which is Pseudomonas aeruginosa. Risk factors include diabetes, advanced age, and immunocompromised status. Clinical features include unremitting otalgia, otorrhea, headache, and cranial nerve involvement. Treatment consists of prolonged parenteral antibiotics and selective surgical debridement. Inadequate treatment of NOE is associated with significant morbidities, in...

Value of SPECT/CT over planar imaging during Tc-99m MDP triple phase bone scintigraphy for detection of osteomyelitis in patients with stage IV pressure ulcers
Conclusion: SPECT/CT imaging has a significant added diagnostic value over planar imaging in accurately detecting osseous involvement with stage IV pressure ulcers. It significantly reduced equivocal results by accurate uptake localization, reducing false positive results due to other associated pathologies and reducing false negative results due to overlap by other structures or difficult patient positioning.

Increased perioperative C-reactive protein and decreased postoperative albumin is associated with acute posttraumatic osteomyelitis in patients with high-energy tibial fractures
Early diagnosis of acute posttraumatic osteomyelitis (POM) is of vital importance for avoiding devastating complications. Diagnosing POM is difficult due to the lack of a highly specific and sensitive test, such as in myocardial infarct, stroke and intracranial bleeding. Serum inflammatory markers, C-reactive protein (CRP), procalcitonin (PCT), white blood cells (WBC) can support clinical findings but they are not able to differentiate between inflammatory response to infection and the host response to non-infection insult with high specificity and sensitivity.
