P153/189 Economic impact of improved functional outcomes associated with the embotrap device vs. trevo retriever and solitaire revascularization device for treatment of acute ischemic stroke: an economic analysis of mastro i from a german hospital perspective
ConclusionThe use of EmboTrap in Germany may lead to reduction in short-term hospital costs, compared to Solitaire or Trevo due to improved patient functional outcomes (90-day mRS 0–2). These findings may inform evidence-based decision making when selecting a cost-efficient SR for MT.Disclosure of InterestOsama Zaidat reports consulting fees for Stryker, Medtronic, Cerenovus, and Penumbra; research grants from Stryker, Medtronic, Cerenovus, Penumbra, and Genentech; in addition, Dr Zaidat had a patent for ischemic stroke issued.Tommy Andersson is a consultant for Anaconda, Cerenovus, Neuravi and Rapid Medical, and hol...

E-104 Inhibiting the neuron-immune axis in acute ischemic stroke reverses microglia-induced neuroinflammation after reperfusion
ConclusionTogether, these findings indicate that ischemic/hypoxic-induced neuronal excitotoxicity promotes neuroinflammation in the setting of reperfusion after ischemic stroke. Inhibiting abnormal neuronal activation triggered by ischemia may prevent microglia-induced neuroinflammation that could protect the brain from secondary brain damage.Disclosures X. Guo: None. D. O-017 Viz LVO versus rapid LVO in detection of large vessel occlusion on computed tomography angiography for acute stroke
ConclusionViz LVO and Rapid LVO had similar NPV, but Rapid LVO produced a significantly larger number of false positive values. False positive values can be a source of alarm desensitization, leading to missed alarms or delayed responses. EF and intracranial atherosclerosis were significant predictors of incorrect predictions in both software packages. We hope this data will be used to improve future algorithms.Disclosures A. E-207 Endovascular thrombectomy versus medical management for large vessel occlusion stroke patients with severe baseline disability: long-term outcomes and transitions to comfort care
ConclusionsAmong patients with severe baseline disability, EVT is associated with less post-stroke accumulated disability without limiting transitions to comfort care. EVT is compatible with goal-concordant care in patients with severe baseline disability; it should not be routinely withheld on the sole basis of baseline disability.Disclosures A. Awad: None. M. P-008 Probabilistic ensemble learning for prediction of stroke thrombectomy outcomes from the neurovascular quality initiative-quality outcomes database (NVQI-QOD) registry
ConclusionsThis study demonstrates the utility of probabilistic ensemble learning in clinical decision-making and prognosis. It can provide robust predictions as well as quantify data uncertainty. Our results regarding NIHSS changes reinforce the substantial benefits of MT, that can improve outcomes in nearly half of patients. The degree of disability relevant to the 90-day follow-up mRS can be determined by probabilistic learning available as early as discharge.Disclosures C. Zhou: None. S. Faruqui: None. A. Patel: None. Slow is smooth, smooth is fast
Modern infantry combat motto. If you can’t move, you get pinned down but if you move too fast you get outflanked and surrounded. The milestone trials1 in 2015 showed that patients with large vessel occlusion can be effectively treated to prevent future disability and, to a certain extent, their mortality. Large vessel occlusion thrombectomy opened the doors of a new era in neurointerventional surgery, in which small departments, often handled by one or two operators, had to pivot and face a steady, continuous increase in the number of procedures two- or threefold their usual capacity. Stroke can also be held accounta...

Patient-reported outcome measures after thrombectomy in patients with acute stroke: fine-tuning the modified Rankin Scale
Conclusions A specifically designed digital platform allows a high collection rate of PROMs among stroke patients who underwent EVT. The mRS score shows a ceiling effect and seems insufficient to fine-tune long-term clinical results. The use of PROMs may allow a better characterization of long-term outcome profiles after EVT.

Benefit of successful reperfusion achieved by endovascular thrombectomy for patients with ischemic stroke and moderate pre-stroke disability (mRS 3): results from the MR CLEAN Registry
Conclusions Although patients with AIS with pre-stroke mRS 3 comprise a heterogenous group of disability causes, we observed improved outcomes when patients achieved successful reperfusion after EVT.

Association between anesthesia modality and clinical outcomes following endovascular stroke treatment in the extended time window
Conclusion LA without sedation during EVT was associated with improved functional outcomes compared with GA, but not CS, within and beyond 6 hours from stroke onset. Prospective studies assessing anesthesia-related outcomes in the extended time window are warranted.
