Usefulness of combining CISS and digital subtraction angiography in diagnosis of isolated posterior inferior cerebellar artery dissection
A 63-year-old man was admitted to our stroke center with brain infarction in the left posterior inferior cerebellar artery (PICA) territory. The initial MRI showed no findings suggestive of arterial dissection, and post-discharge MRI showed no temporal changes. Digital subtraction angiography (DSA) revealed vasodilation of the proximal portion of the PICA but it was uncertain whether dissection was present. Discrepancy between the outer contour seen on constructive interference in steady state (CISS) MRI and the inner contour seen on DSA suggested the presence of intramural hematoma.

Percutaneous atrial shunt closure and the risk of recurrent ischemic stroke: A register-based, nationwide cohort study
Patent foramen ovale may cause paradoxical emboli1. Cryptogenic ischemic cerebrovascular events (stroke or transient ischemic attack) (CVE) accounts for about 25% of ischemic strokes.2,3 Transcatheter closure of atrial shunts, including atrial septal defects (ASD) and patent foramen ovale (PFO), was introduced in 1996.4-6 Initially,randomized studies failed7-9 but more recent randomized studies have shown superiority of interventional treatment, with 68 % reduction of recurrent stroke after transcatheter closure of PFO compared with medical treatment in patients with cryptogenic CVE.

Globally, there are estimated to be almost 25.7 million stroke survivors, and stroke is the third most common cause of disability worldwide.1 The impact of stroke is sudden, and can be devastating and life-changing.2 A stroke can result in a range of short and long-term physical, psychological and social problems for survivors.3 –6 Stroke survivors’ needs can present at different times post-stroke and differ between individuals. Evidence suggests that, long-term, stroke survivors’ needs are often inadequately met.

Stroke epidemiology varies among different populations. Ethnicity, socioeconomic factors and local standards for risk factors management may explain, at least in part, this phenomenon.1 Burden of stroke is particularly high in low- and middle-income countries such as those in Latin America2, and it is expected to grow driven by aging of the general population, the high prevalence of vascular risk factors and overall declining stroke mortality.3 Latin America is a diverse region in terms of social, cultural and economic aspects.

Prevalence and prognosis of seizures among patients undergoing mechanical thrombectomy for acute ischemic stroke: A look at pre-2015 aha/asa guidelines update regarding endovascular treatment
Mechanical Thrombectomy (MT) is standard of care for eligible patients with Acute Ischemic Stroke (AIS) due to large vessel occlusion (LVO). With increasing use of MT, clinicians are more likely to encounter seizures, a potential complication of AIS treated with MT. Tracking future trends in the burden of post-stroke seizure associated with MT will require baseline pre-approval benchmark estimates of its frequency and outcomes.

Mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes diagnosed after metformin-triggered stroke-like episodes
A 40-year-old man with sensorineural hearing loss and diabetes mellitus was hospitalized with acute-onset impaired consciousness and clumsiness in his left hand. He had been taking metformin (750 mg/day) for 4 months. He had no febrile illness, energy failure, or use of mitochondria-toxic drugs other than metformin. The patient had a family history of diabetes mellitus (maternal uncle and aunt). His weight and height were 35 kg and 1.58 m, respectively. A neurological examination revealed confusion and weakness in the left upper limb.

The effects of aneurysmal subarachnoid hemorrhage on cerebral vessel diameter and flow velocity
Transcranial Doppler flow velocity is used to monitor for cerebral vasospasm following aneurysmal subarachnoid hemorrhage. Generally, blood flow velocities appear inversely related to the square of vessel diameter representing local fluid dynamics. However, studies of flow velocity-diameter relationships are few, and may identify vessels for which diameter changes are better correlated with Doppler velocity. We therefore studied a large retrospective cohort with concurrent transcranial Doppler velocities and angiographic vessel diameters.

Intravenous thrombolysis for mild stroke: NIHSS 3 –5 Versus NIHSS 0–2
Studies have shown that mild stroke patients with National Institutes of Health Stroke Scale (NIHSS) score 3 –5 but not 0–2 may benefit from the intravenous thrombolysis when compared with antiplatelet therapy. We aimed to compare the safety and effectiveness of thrombolysis in mild stroke with NIHSS score of 0–2 vs. 3–5 and identify the predictors of an excellent functional outcome in a real world longitudinal registry.

Postoperative stroke assessment inconsistencies in cardiac surgery: Contributors to higher stroke-related mortality?
Postoperative stroke remains a devastating complication of cardiac surgery and is associated with higher rates of postoperative morbidity and mortality.1 Postoperative stroke occurs after cardiac surgery in ≈ 1-5% of patients owing to high rates of atherosclerotic disease, pre-existing cerebrovascular comorbidities, dysrhythmias, and elevated age. Paradoxically, in-hospital strokes are associated with 3-10-fold higher mortality than out of hospital strokes,2,3 potentially due to delays in diagnosis a nd greater medical complexity complicating treatment options.
