Imaging the aortic arch to improve diagnostic and prognostic accuracy in transient ischemic attack patients
This study raises a final relevant question: since the prognosis of TIA patients worsens in proportion to the severity of AAA, what therapeutic options are available for these patients? The clinical guidelines recommend risk factor modification and therapy with statins (Class IIa, level of evidence C), an approach consistent with treatment of any patient affected by atherosclerosis. Strokes, cholesterol and statins: When mortality is an endpoint
The Yi et al. stroke mortality study, with 5.2 million person-years of observation in a representative Korean population cohort with a single baseline total cholesterol (TC) measurement, highlights several important issues [1]. First, the authors note the fact that cholesterol lowering “particularly by statins” does not lower stroke mortality in randomized trials. Second, the fact that 53% of stroke deaths were from hemorrhagic strokes, far surpassing its incidence of about 10–20% in all strokes in many countries [1], and that are mainly nonfatal ischemic strokes.

Effect of initiating statin therapy on long-term outcomes of patients with dyslipidemia after intracerebral hemorrhage
Intracerebral hemorrhage (ICH) has a higher mortality than ischemic stroke. Statin is beneficial for stroke, but high potency statin treatment has been associated with the risk of hemorrhagic stroke. The aim of this study was to assess the impact of initiating statin therapy after ICH on cardiovascular outcomes.

Leukocytes, platelets and cardiovascular diseases
We read with great interest the recent article in Atherosclerosis by Xu et al. [1]. In a large prospective study comprising 13,929 middle age and elderly individuals of Chinese descent, they showed that the systemic immune-inflammation index (SII), the product of the neutrophil count x platelet count x lymphocyte count, predicted total stroke, ischemic stroke and hemorrhagic stroke over a median follow-up of 8.3 years using multivariate Cox regression analyses [1]. Although there was a threefold increase in coronary heart disease (CAD), SII did not predict CAD or acute coronary syndromes (ACS).

Carotid intima-media thickness for cardiovascular risk assessment: Systematic review and meta-analysis
Conclusions: CIMT as measured by B-mode ultrasound is associated with future cardiovascular events. However, the addition of CIMT to traditional cardiovascular risk prediction models does not lead to a statistical significantly increase in performance of those models.

Serum total adiponectin level and the risk of cardiovascular disease in general population: A meta-analysis of 17 prospective studies
Conclusions: Increased serum adiponectin was related to an elevated risk of ischemic stroke, but there was no clear evidence indicating a positive relationship between adiponectin and the risk of CHD or CVD.

Parental history and the risk of subsequent vascular events in patients with clinically manifest vascular disease: The effects of sex of the parent and vascular disease location
Conclusions: A positive parental history of cardiovascular disease without information about vascular disease location and sex does not increase the risk of recurrent vascular events in patients with symptomatic vascular disease.
