Another Step Forward in Our Ability to Predict Percutaneous Coronary Intervention Outcomes
Despite advancements in devices, technique, and patient selection, adverse events including bleeding, vascular complications, myocardial infarction, stroke, and death still occur during percutaneous coronary intervention (PCI). Accurate and up-to-date risk-adjusted models that predict mortality after PCI are important to inform clinical decision making, identify patient factors that increase risk, provide a standardized format to measure programmatic quality, and drive continuous quality improvement efforts to improve operator outcomes.

Direct Oral Anticoagulants: Patients Benefit When Prescribers Get the Message
Direct oral anticoagulants (DOACs) have demonstrated similar or better efficacy to warfarin in reducing stroke and systemic embolic events in large-scale prospective trials.1 –4 In addition, these trials demonstrated either noninferiority or superiority in reducing bleeding events compared with warfarin. To treat high-risk patients with atrial fibrillation (AF) and avoid bleeding in low-risk patients, current guidelines recommend oral anticoagulants in high-risk patien ts with AF with CHA2DS2VASc scores of ≥2 in men and ≥3 in women.

Prevalence and Outcomes of Patients With Acute Ischemic Stroke and Concomitant Non –ST-Elevation Myocardial Infarction (Results from the National Inpatient Sample 2016 to 2019)
We examined hospitalized patients with acute ischemic stroke and a concomitant NSTEMI diagnosis who were included in the National Inpatient Sample 2016 to 2019. Acute ischemic stroke and NSTEMI were defined by using the Inter national Classification of Diseases, Tenth Revision codes. Patients with ST-elevation MI were excluded.

Outcomes of Concomitant Coronary Artery Bypass Grafting in Patients With Infective Endocarditis: A Systematic Review and Meta-Analysis
It is current practice to perform concomitant coronary artery bypass grafting (CABG) in patients with infective endocarditis who have relevant coronary artery disease. However, CABG may add complexity to the operation. We performed a systematic review and a meta-analysis of studies that presented outcomes from patients who underwent valve surgery because of infective endocarditis with or without concomitant CABG. Three databases were assessed. Perioperative mortality was the primary outcome. Long-term mortality and postoperative stroke were the secondary outcomes.

Trends in Cardiovascular Mortality in Patients With Atrial Fibrillation and Cancer in the United States (1999 to 2020)
Cancer has been identified as an independent risk factor for atrial fibrillation (AF). Although the development of novel therapeutic agents has led to significant improvement in cancer-related outcomes, there is also a rising concern regarding the long-term impact of treatment-emergent cardiac adverse events such as AF.1 AF portends a poor prognosis in patients with cancer and is also associated with a twofold increase in the risk of stroke as compared with patients with AF without cancer.2 However, population-level data on the impact of AF on overall cardiovascular mortality in patients with cancer remain limited.

Meta-Analysis of the Outcomes of Peri-Device Leak After Left Atrial Appendage Closure
Left atrial appendage closure (LAAC) reduces the risk of thromboembolic stroke in atrial fibrillation. Peri-device leak (PDL) after LAAC may affect the subsequent risk of thromboembolism. We conducted a systematic review and meta-analysis to evaluate the effect of PDL after LAAC. We searched PubMed/Medline, Embase, and Google Scholar for studies reporting outcomes of PDL after LAAC from inception through October 2022. The primary outcome was the composite of stroke, transient ischemic attack (TIA), or systemic embolism (SE).

Temporal Trends in Risk Factors of Periprocedural Stroke in Patients Undergoing Percutaneous Coronary Intervention: Insights from the ACC NCDR CathPCI Registry
Periprocedural stroke complicates up to 1% of patients undergoing percutaneous coronary intervention (PCI) and is associated with increased morbidity, mortality, and subsequent health care resource utilization.1 –5 Contemporary reports suggest that despite iterative procedural evolution in PCI (e.g., smaller catheters, radial access, and potent pharmacotherapy), periprocedural stroke rates have not improved over the last few years, with our recent analysis suggesting these rates may be in fact be rising, likely as a result of an increasingly co-morbid population3 and higher procedural complexity.

Antithrombotic Medication and Major Complications After Mechanical Aortic Valve Replacement
Patients with mechanical aortic valve replacement (AVR) require lifelong vitamin K antagonist (VKA) therapy for stroke and systemic embolism prevention. However, VKA treatment predisposes patients to various types of bleeding. In the present study, we sought to assess the success of antithrombotic therapy and the occurrence and timing of strokes and bleeding events after mechanical AVR. A total of 308 patients who underwent isolated mechanical AVR were included in the study, and follow-up data were completed for 306 patients (99.4%).

Effect of Concomitant Cardiac Arrest on Outcomes in Patients With Acute Coronary Syndrome-Related Cardiogenic Shock
Patients with acute coronary syndrome (ACS)-related cardiogenic shock (CS) with or without concomitant CA may have disparate prognoses. We compared clinical characteristics and outcomes of patients with CS secondary to ACS with and without cardiac arrest (CA). Between 2014 and 2020, 1,573 patients with ACS-related CS with or without CA who underwent percutaneous coronary intervention enrolled in a multicenter Australian registry were analyzed. Primary outcome was 30-day major adverse cardiovascular and cerebrovascular events (MACCE) (composite of mortality, myocardial infarction, stent thrombosis, target vessel revascularization and stroke).

Lifestyle factors such as unhealthy diets, physical inactivity, tobacco use, and excessive alcohol consumption are closely associated with CVDs.
