Association of Frailty Status on the Causes and Outcomes of Patients Admitted With Cardiovascular Disease
Data are limited about the contemporary association between frailty and the causes and outcomes of patients admitted with cardiovascular diseases (CVD). Using the US National Inpatient Sample, CVD admissions of interest (acute myocardial infarction, ischemic stroke, atrial fibrillation (AF), heart failure, pulmonary embolism, cardiac arrest, and hemorrhagic stroke) were stratified by Hospital Frailty Risk Score (HFRS). Logistic regression was used to determine adjusted odds ratios (aORs) and 95% confidence intervals (CIs) of in-hospital mortality among different groups with frailty.

Effect of Anticoagulation Duration on Stroke Incidence in Asian Patients With Left Ventricular Thrombus
Left ventricular thrombus (LVT) occurs in acute myocardial infarction (AMI) and in ischemic and nonischemic cardiomyopathies.1,2 LVT may result in embolic stroke.3 We read with interest the study by Lorente-Ros et  al4 published in the American Journal of Cardiology. That study, conducted in a Spanish cohort, found that prolonged anticoagulation>12  months for LVT was associated with lower stroke risk. We aimed to study the generalizability of these results in an Asian population because the original article was a single-center study with limited representation.

Impact of Age at the Time of the First ST-Elevation Myocardial Infarction on 10-Year Outcomes (from the EXAMINATION-EXTEND Trial)
The aim of this substudy of the EXAMINATION-EXTEND was to analyze 10-year outcomes according to the patient's age at the time of the first ST-elevation myocardial infarction (STEMI). Of 1,498 patients with STEMI included in the EXAMINATION-EXTEND study, those with a previous history of coronary ischemic even or ischemic stroke were excluded from this analysis. The remaining 1,375 patients were divided into 4 age groups:75  years. The primary end point was 10-year patient-oriented composite end point (POCE) of all-cause death, any MI, or any revascularization.

Transcatheter Patent Foramen Ovale Closure in Patients With Transient Ischemic Attack
The objectives of this study were to determine the clinical and procedural characteristics and long-term outcomes of patients with TIA who underwent transcatheter PFO closure. This was a multicenter study including 1,012 consecutive patients who underwent PFO closure after a cerebrovascular event. Patients were divided into 2 groups according to their index event leading to PFO closure: TIA (n  = 183 [18%]), and stroke (n = 829 [82%]).

The “Heart” of Environmental Justice
Environmental exposures, especially air pollution, have been linked with heart disease.1 Air pollution alone is estimated to cause 6 to 9 million deaths globally annually, mostly attributable to ischemic heart disease and stroke.1 The health impact of environmental exposures varies significantly depending on the susceptibility and vulnerability of the exposed populations.2 Underrepresented minorities and people of color often face higher exposures to various pollutants, owing to historic inequities and structural racism, which impact cardiovascular disease disparities.

Advanced Echocardiography With Left Atrial Strain and Indexed Left Atrial Three-Dimensional Volume for Predicting Underlying Atrial Fibrillation After Cryptogenic Stroke
Cryptogenic stroke (CS) represents 1/3 of ischemic strokes. Atrial fibrillation (AF) can be detected in up to 30% of CS. Therefore, there is a clinical need for predicting AF to guide the optimal secondary prevention strategy. The evidence about the role of advanced echocardiography, including left atrial 3-dimensional (3D) index volume and left atrial strain (LAS) techniques, to predict underlying AF in this setting is lacking. From April 2019 to November 2021, 78 consecutive patients with ischemic stroke or transient ischemic attack with ABCD2 scale ≥4 of unknown etiology were prospectively recruited.

Stroke Risk Stratification With the CHADS2 Score in Patients Without Atrial Fibrillation Who Underwent Percutaneous Coronary Intervention
The clinical significance of the CHADS2 score remains unclear in patients with coronary artery disease (CAD) without atrial fibrillation (AF). Therefore, the purpose of this study was to evaluate the association between the CHADS2 score and the long-term risk of ischemic stroke and its severity in patients with CAD with and without AF. Using the CREDO (Coronary Revascularization Demonstrating Outcome study)-Kyoto Registry Cohort-3, the present study population consisted of 11,516 patients with CAD who underwent percutaneous coronary intervention without oral anticoagulants at discharge.

Controlled Level and Variability of Systolic Blood Pressure on the Risk of Thromboembolic Events in Atrial Fibrillation and Hypertension
Hypertension is an independent risk factor for thromboembolic events in patients with atrial fibrillation (AF). However, the association between blood pressure (BP) control and thromboembolic events remains under-evaluated in patients with AF. We aimed to identify the relation between BP control and the risk of ischemic stroke and systemic embolism in hypertensive patients with AF. Influence of the Danish Co-morbidity Index Score on the Treatment and Outcomes of 2.5 Million Patients Admitted With Acute Myocardial Infarction in the United States
This study aimed to determine the association between the Danish Co-morbidity Index for Acute Myocardial Infarction (DANCAMI) and restricted DANCAMI (rDANCAMI) scores and clinical outcomes in patients hospitalized with AMI. Using the National Inpatient Sample, all AMI hospitalizations were stratified into four groups based on their DANCAMI and rDANCAMI score (0; 1 to 3; 4 to 5; ≥6). The primary outcome was all-cause mortality, whereas secondary outcomes were major adverse cardiovascular/cerebrovascular events, major bleeding, ischemic stroke, and receipt of coronary angiography or percutaneous coronary intervention.

Impact of Cerebral Embolic Protection Devices on the Incidence and Outcomes of Delirium After Transcatheter Aortic Valve Implantation
Postoperative delirium is a well-recognized complication after transcatheter aortic valve implantation (TAVI) that is associated with a significantly increased mortality and prolonged hospitalization stay.1 Ischemic brain injury has been hypothesized to be one of the predominant mechanisms responsible for delirium after TAVI. Brain injury during TAVI involves microinfarctions caused by cerebral embolization of the aortic plaque or valve particles dislodged during the prosthesis deployment.1,2 The role of cerebral embolic protection (CEP) devices has been studied in preventing stroke during TAVI.
