Catheter ablation for patients with atrial fibrillation and heart failure with reduced and preserved ejection fraction: insights from the KiCS-AF multicentre cohort study
ConclusionAmong AF-HF patients, CA was associated with improved QoL and lower risk of cardiovascular events against drug therapy alone, even for patients with mildly reduced and preserved LVEF.

Dose adjustment based on the plasma level measurement used anti-FXa chromogenic assay on the use of non-vitamin K antagonist oral anticoagulants (NOACs) improved the overall benefit of NOACs dosing in patients with atrial fibrillation: Authors ’ reply
We thank the authors for alerting us to their recently published article on non-vitamin K antagonist oral anticoagulant (NOAC) plasma level assessments.1 Indeed, despite extensive literature review, this interesting and important article has slipped our attention.2 We like to congratulate the authors for their detailed and well-represented work. Based on our 2018 EHRA Practical Guide ’s recommendations,3 Suwaet al. measured plasma levels in both patients on rivaroxaban and apixaban and found acceptable peak plasma levels even in a substantial proportion of patients with off-label dose reductions. Especially in patients w...

2021 European Heart Rhythm Association Practical Guide on the use of non-vitamin K antagonist oral anticoagulants in patients with atrial fibrillation: comment —Authors’ reply
We thank the authors for addressing this important issue.1 Indeed, the assessment of NOAC plasma levels —or potential alternatives—in special situations may represent an important element in the further refinement of non-vitamin K antagonist oral anticoagulant (NOAC) therapy. In their letter, Mbroh and Poli make a case for using point-of-care coagulation tests (POCT) to more rapidly assess the lev el of anticoagulant in patients’ blood samples. In the 2021 EHRA Practical Guide on the use of NOACs, we suggest to consider NOAC plamsa level assessment in two different types of scenarios: emergency situations (e.g. acute...

2021 European Heart Rhythm Association Practical Guide on the use of non-vitamin K antagonist oral anticoagulants in patients with atrial fibrillation: comment
We have read with keen interest the updated practical guide by Steffelet al.1 For safer decision-making, we share the authors ’ view on the need for rapid assessment of anticoagulation in patients under non-vitamin K antagonist oral anticoagulant (NOAC) during emergencies such as bleeding and urgent surgical intervention. Our group has been involved in studies on point-of-care coagulation testing (POCT) in acute stroke p atients where rapid decision-making with respect to thrombolysis and anticoagulation reversal is vital.

Implications of atrial fibrillation on the clinical course and outcomes of hospitalized COVID-19 patients: results of the Cardio-COVID-Italy multicentre study
ConclusionAmong hospitalized patients with COVID-19, a history of AF contributes to worse clinical course with a higher mortality and in-hospital events including new-onset AF, acute HF, and multiorgan failure. The mortality risk remains significant after adjustment for variables associated with COVID-19 severity and comorbidities.

A meta-analysis of clinical risk factors for stroke in anticoagulant-na ïve patients with atrial fibrillation
ConclusionThis study confirms age and prior stroke as the strongest predictors of stroke or systemic embolism in anticoagulant-naive AF patients. Other predictors include hypertension, diabetes, heart failure, and vascular disease. Female sex seems not to be universally associated with stroke or systemic embolism.

The Atrial Fibrillation Better Care pathway for managing atrial fibrillation: a review
AbstractThe 2020 European Society of Cardiology guidelines endorse the Atrial Fibrillation Better Care (ABC) pathway as a structured approach for the management of atrial fibrillation (AF), addressing three principal elements: ‘A’ – avoid stroke (with oral anticoagulation), ‘B’ – patient-focused better symptom management, and ‘C’ – cardiovascular and comorbidity risk factor reduction and management. This review summarizes the definitions used for the ABC criteria in different studies and the impact of ad herence/non-adherence on clinical outcomes, from 12 studies on seven different cohorts. All studies co...

Real-world experience with the insertion of a new implantable cardiac monitor
ConclusionThe new BM III was inserted in typically less than 5 minutes until wound closure. A relevant number of insertions took place in a consultation room. Prophylactic antibiotics may be unnecessary, because no pocket infections were reported, although no antibiotic prophylaxis was used in one half of all cases (N = 229). In summary, the insertion with the new tool is fast and has a low risk of complications.

Main determinants of physician-driven amiodarone discontinuation in clinical practice
ConclusionStudy showed that within the first two years of treatment, despite persistent indication, amiodarone was discontinued in 1 out of 10 patients in the absence of side effects, mostly in younger patients with less comorbidities, which may not always be justified. There is a need for qualitative research to elucidate the reasons for such physicians ’ decisions. Electrocardiographic markers of incident atrial fibrillation in patients with cryptogenic stroke
Conclusion: PWD and P-wave axis predicted incident AF in this subset of pts with cryptogenic stroke. The ECG may be a toll to identify pts at risk of developing AF, although larger studies are needed to confirm these results. Abstract Figure.

Incidence and type of arrhythmias recorded by one-month continuous ECG monitoring in stroke patients
CONCLUSIONSAF and other potentially relevant arrhythmias are frequent after stroke and easily detectable with one-month non-invasive continuous ECG monitoring. Patients with a parafibrillatory status could benefit from longer monitoring time to detect AF.

Antithrombotic treatment management in low stroke risk patients undergoing cardioversion of atrial fibrillation & lt;48 h duration: results of an EHRA survey
AbstractData supporting the safety of cardioversion (CV) of atrial fibrillation (AF) without anticoagulation in patients with AF duration<48  h are scarce. Observational studies suggest that the risk of stroke in these patients is very low when the definite duration of the AF episode is of<48  h and the clinical risk profile as estimated through the CHA2DS2VASc score is low (a score of 0 for men and 1 for women). As the recent 2020 European Society of Cardiology (ESC) guidelines indication for this clinical scenario is based mainly on consensus, we sent out a survey to assess the current clinical practice on anti...

Characteristics and clinical outcomes in atrial fibrillation patients classified using cluster analysis: the Fushimi AF Registry
ConclusionsWe identified six clinically relevant phenotypes of AF patients on cluster analysis. These phenotypes can be associated with various types of comorbidities and associated with the incidence of clinical outcomes.Clinical Trial Registration Informationhttps://www.umin.ac.jp/ctr/index.htm.
