15 Early prolonged ambulatory cardiac monitoring in stroke (EPACS) – an open-label randomised controlled trial
Introduction: Cardioembolism in paroxysmal atrial fibrillation (PAF) is a preventable cause of transient ischaemic attack (TIA) or ischaemic stroke, however, due its transient nature, a short-duration Holter monitor may miss a significant proportion of events. There are current limitations in ECG recording beyond 3-7 days. While implantable loop recorders require a minor surgical procedure, event-triggered loop recorders are cumbersome to use. We investigated whether there was a role for early prolonged cardiac monitoring using a novel patient-friendly cardiac monitoring system on patients presenting early after a stroke or TIA.

17 Atrial arrhythmia detection by implantable loop recorder in patients with and without previous stroke; implications for investigating patients with unexplained ischaemic stroke
Background/Introduction: Among patients with unexplained ischaemic stroke, prolonged cardiac rhythm monitoring with an Implantable Loop Recorder (ILR) has been found to uncover a high incidence of Atrial Fibrillation (AF). However, the incidence of AF detected by ILR in the general population is not known, making it possible that AF detected among stroke patients simply reflects background “noise.” It is also unclear what burden of AF is required to increase stroke risk and whether short bursts of AF are truly pathologic or simply normal phenomena, detected in the general population.

Age and sex differences in atrial fibrosis among patients with atrial fibrillation
ConclusionsAdvancing age and female sex are associated with a higher burden of atrial fibrosis in patients with AF. Women with a prior history of stroke also have higher fibrosis compared with both women and men without history of stroke. Advanced fibrosis may explain the female and age association with stroke in AF.

The influence of progression of atrial fibrillation on quality of life: a report from the Euro Heart Survey
ConclusionAF progression is associated with a decrease in HRQoL. However, multivariate analysis revealed that AF progression itself does not have a negative effect on HRQoL, but that this effect can be attributed to a minor effect of the associated symptoms and a major effect of associated adverse events.

Epicardial left atrial appendage AtriClip occlusion reduces the incidence of stroke in patients with atrial fibrillation undergoing cardiac surgery
ConclusionThe long-term results from our first-in-man prospective human trial plus our institutional registry of epicardial LAA occlusion with the AtriClip in patients with AF undergoing cardiac surgery demonstrate the safety and durability of the procedure. In addition, our data are suggestive for the potential efficacy of LAA occlusion in reducing the incidence of stroke. If validated in future large randomized trials, routine LAA occlusion in patients undergoing cardiac surgery (with contraindications to treatment with oral anticoagulants) may represent a reasonable adjunct procedure to reduce the risk of future stroke....

Combining Watchman left atrial appendage closure and catheter ablation for atrial fibrillation: multicentre registry results of feasibility and safety during implant and 30 days follow-up
ConclusionThe outcomes from these international, multicentre registries support the feasibility and safety of performing combined procedures of ablation and Watchman LAAC for patients with non-valvular AF and high stroke risk. Further data are needed on long-term outcomes for the hybrid technique on all-cause stroke and mortality.

Symptomatic atrial fibrillation and risk of cardiovascular events: data from the Euro Heart Survey
ConclusionPersistence or development of symptoms after medical treatment are associated with an increased risk of CV events during a 1-year follow-up. Type of AF, along with hypothyroidism, COPD and chronic heart failure are significantly associated with symptoms persistence despite medical treatment.

Dabigatran vs. warfarin in relation to the presence of left ventricular hypertrophy in patients with atrial fibrillation — the Randomized Evaluation of Long-term anticoagulation therapY (RE-LY) study
ConclusionsLVH was associated with a lower antithrombotic efficacy of warfarin, but not of dabigatran, in patients with AF. Consequently, the relative benefit of the lower dose of dabigatran compared to warfarin was enhanced in patients with LVH. Long-term clinical impact of permanent cardiac pacing after transcatheter aortic valve implantation with the CoreValve prosthesis: a single center experience
