Macrocalcification of intracranial vertebral artery may be related to in-stent restenosis: lessons learned from optical coherence tomography
Conclusions This study suggests a potential relationship between macrocalcifications and the risk of in-stent restenosis of the intracranial vertebral artery. These preliminary findings obtained from a limited sample should be verified by prospective large-scale studies.

Lessons From Flint, Mich. on Stroke Preparedness and Lytic Uptake
(MedPage Today) -- DALLAS -- Efforts to boost stroke treatment in Flint, Michigan had mixed results that nevertheless left the community with one of the highest thrombolytic usage rates in the state, even amid a public health crisis, according...

Influence of Combined Transcranial Direct Current Stimulation and Motor Training on Corticospinal Excitability in Children With Unilateral Cerebral Palsy
Conclusion This proof-of-principle study evaluated the influence of cathodal contralesional tDCS on corticospinal excitability in pediatric participants with UCP. A hypothesized decrease in contralesional excitability was noted in participants in the Active+CIMT group, however, the efficacy of tDCS to modulate corticospinal excitability was not statistically different than the Sham+CIMT group. A more detailed understanding of how tDCS impacts M1 neurophysiology will be essential to inform future clinical trials on the optimal dosing parameters, based on individual brain circuitry, to explore the potential functional benef...

Boehringer Aims High with Angels Initiative
An innovative scheme to create a community of 1,500 stroke-ready hospitals across Europe has been recognized with the Most Valuable HCP/Healthcare Award at the European arm of theeyeforpharma Awards 2017.The Angel ’s Initiative from Boehringer Ingelheim – which aims to save 100,000 lives by May 2019 – was voted the winner in a unanimous decision by the expert panel of judges.Only one-third of stroke patients in Europe have access to organized stroke care, while just one per cent of stroke patients in emerging markets are provided with lifesaving treatments. In response, the Angels Initiative is working to establish m...

A case report of parenchymal hematoma after intravenous thrombolysis in a rivaroxaban-treated patient: Is it a true rivaroxaban hemorrhagic complication?
AbstractRationale:To date, the only treatment approved for acute ischemic strokes is thrombolysis. Whether intravenous thrombolysis may be safe in patients taking direct oral anticoagulants (DOACs) is currently a matter of debate.Patient concerns:A 74-year-old woman, who was on rivaroxaban 20 mg/d for nonvalvular atrial fibrillation, was admitted to our stroke unit with left-sided hemiparesis and aphasia. The onset of neurologic deficits had occurred 5 hours after the last rivaroxaban dose.Diagnosis:An acute ischemic stroke was diagnosed.Interventions:The patient was administered thrombolytic treatment with intravenous r...

Combined treatment of botulinumtoxin and robot-assisted rehabilitation therapy on poststroke, upper limb spasticity: A case report
This study investigated whether BoNT-A injection combined with robot-assisted upper limb therapy improves voluntary motor control or functions of upper limbs after stroke.Patient concerns:Two subacute stroke patients were transferred to the Department of Rehabilitation.Diagnoses:Patients demonstrated spasticity in the upper extremity on the affected side.Interventions:BoNT-A was injected into the paretic muscles of the shoulder, arm, and forearm of the 2 patients at the subacute stage. Conventional rehabilitation therapy and robot-assisted upper limb training were performed during the rehabilitation period.Outcomes:Manual ...

Results of carotid artery stenting. Lessons learned in a Belgian 'real world' practice
CONCLUSION: Real world data from a low volume center show that CAS performed in patients with high risk for CEA yields acceptable outcome that is comparable to the literature. Since CAS is a delicate procedure we advice to centralize the procedure to an dedicated experienced interventionalist and to perform rigorous quality control of your 'real world' data.PMID:33820485 | DOI:10.1080/00015458.2021.1911750

Thrombolysis and Expedited Carotid Revascularization.
Abstract With the move towards expedited carotid endarterectomy (CEA) in patients presenting with a TIA/minor stroke and for offering intravenous thrombolysis <3 hours to patients with acute ischemic stroke, it is inevitable that surgeons will be asked to consider CEA in patients who have made a good recovery from their stroke after thrombolysis and who have a 50-99% ICA stenosis. The key question is; 'how long after thrombolysis should I delay CEA to minimise the peri--operative risk (especially intracranial hemorrhage (ICH)), whilst also minimizing the risks of further embolization and stroke?' Thirteen serie...

The Most Beautiful Dance I Ever Enjoyed With My Husband
This past weekend my husband, Dan danced at our friend's daughter's wedding. Well, if you consider swaying from side to side dancing, then it qualifies. While it may not seem so remarkable, the fact that Dan wiggled on a dance floor is amazing and wonderful! It was just five years ago that his life took a cruel turn and he suffered a devastating stroke. He couldn't walk, talk or eat. He had a feeding tube in his stomach, and we were told he was "gravely" ill. The stroke had affected his brain stem where bodily functions were regulated, paralyzed the optic nerve and traveled beyond. He received tPA -- tissue plasminogen ac...

Acute endovascular recanalization: lessons from randomized controlled trials
Purpose of review: The purpose is to review the results and impact of recent positive studies on endovascular stroke treatment in the context of previous negative trials. Recent findings: Since October 2014, the results of five randomized controlled multicenter trials on the endovascular stroke treatment as adjunct to conservative treatment (largely including intravenous tissue-type plasminogen activator) versus conservative treatment alone have been published. All of these trials largely used stent retrievers as endovascular device and included patients with proven large vessel occlusion (mostly distal internal carotid o...

Quick needle insertion at pharyngeal acupoints for poststroke dysphagia: A case report
We present a case report describing a new treatment for dysphagia, namely, quick needle insertion at pharyngeal acupoints. Patient concerns: A 70-year-old man developed pharyngeal dysphagia after a stroke. Three months after the patient experienced a sudden stroke leading to liquid dysphagia, acupuncture, one of the most important therapies in Traditional Chinese Medicine, was used to treat the patient. Diagnoses: A diagnosis of cerebral infarction and bulbar paralysis was made. Interventions: Quick needle insertion was performed at five pharyngeal acupoints, once a day, 6 times a week for 6 weeks. Outcomes: Th...
