O-042 Complications of DAPT cessation at 3 months versus 6 months after flow diversion of unruptured intracranial aneurysms
ConclusionsOnly 0.6% (n=1) of patients exhibited complications after 3 month follow-up. We conclude that DAPT duration of 3 months after endoluminal flow diversion of unruptured cerebral aneurysms of the anterior circulation is not associated with increased rates of hemorrhagic or thromboembolic complications compared to a traditional 6-month regimen.Disclosures S. E-033 Optimization of endovascular treatment for patients with acute carotid plaque rupture and free- floating thrombus by use of two overlapping closed-cell stents with different radial force
ConclusionUse of the Carotid Wallstent as the first stent to secure the free-floating thrombus allows save delivery of the Xact stent which then adds a second closed-cell mesh layer as well as the needed radial force to expand the stent construct. This approach may decrease if not eliminate the need for post-stent angioplasty in the acute setting thus decreasing risk of thromboembolic complication.Disclosures A. Kuhn: None. J. Singh: None. S. Aspirin and clopidogrel resistance; a neglected gap in stroke and cardiovascular practice in Iran: a systematic review and meta-analysis
Antiplatelet drugs, such as Aspirin and Clopidogrel (Plavix) are effective in the primary prevention of thromboembolic events. They are commonly used to reduce the risk of recurrence of thromboembolism. The bo...

Clopidogrel Is Associated with Reduced Likelihood of Aneurysmal Subarachnoid Hemorrhage: a Multi-Center Matched Retrospective Analysis
We present, to the best of our knowledge, the first large-scale multi-institutional analysis suggesting clopidogrel use is protective against intracranial aneurysm rupture. It is our hope that these data will guide future investigation, revealing the pathophysiologic underpinning of this association.

Acute coronary syndrome in very elderly patients —a real-world experience
AbstractVery elderly population constitutes an increasingly larger proportion of patients admitted for acute coronary syndromes (ACS). Notably, age represents both a proxy of frailty and an exclusion criterion in clinical randomized trials, which probably contributes to lack of data and undertreatment of real-world elderly patients. The aim of the study is to describe patterns of treatment and outcome of very elderly patients with ACS. All consecutive patients aged  ≥ 80 years old (yo) admitted between January 2017 and December 2019 with ACS were included. The primary endpoint was in-hospital occurrence of major adv...

The contribution  of genotype-guided selection of P2Y12 inhibitor on prognosis in ACS /CCS patients undergoing percutaneous coronary intervention: a retrospective cohort study
ConclusionsGenotype-guided selection of P2Y12 inhibitor made a very positive contribution on the prognosis in Chinese ACS/CCS patients undergoing PCI. Instead of intensifying antiplatelet strategies, conventional-dose clopidogrel could be recommended as P2Y12 inhibitor after weighing MACEs and bleeding events in CYP2C19 IM patients.

Ischemic Stroke Management: Posthospitalization and Transition of Care
Am Fam Physician. 2023 Jul;108(1):70-77.ABSTRACTIschemic stroke is a major cause of morbidity and mortality worldwide. Ischemic stroke and transient ischemic attack exist on a continuum of the same disease process. Ischemic stroke is common, and more than 85% of stroke risk is attributed to modifiable risk factors. The initial management of acute stroke is usually performed in the emergency department and hospital settings. Family physicians have a key role in follow-up, ensuring that a complete diagnostic evaluation has been performed, addressing modifiable risk factors, facilitating rehabilitation, and managing chronic s...

Which Antiplatelet Agent for Patients With TIA and Obesity? Which Antiplatelet Agent for Patients With TIA and Obesity?
Ticagrelor plus aspirin was associated with a lower risk for recurrent stroke than clopidogrel plus aspirin in patients with obesity who carry the CYP2C19 loss-of-function allele.Medscape Medical News
