Original Article: Increased Stroke Severity and Mortality in Patients with SARS-CoV-2 Infection: An Analysis from the N3C Database
Studies from early in the COVID-19 pandemic showed that patients with ischemic stroke and concurrent SARS-CoV-2 infection had increased stroke severity. We aimed to test the hypothesis that this association persisted throughout the first year of the pandemic and that a similar increase in stroke severity was present in patients with hemorrhagic stroke.

Factors associated with stroke in patients with paroxysmal atrial fibrillation beyond CHADS2 score.
CONCLUSIONS: Elevated E velocity, E/E' and SD of tA-SR were associated with occurrence of stroke in patients with PAF even with similar CHAD scores. Increased SD of tA-SR and E/E' were independently associated with stroke in patients with PAF. PMID: 27296157 [PubMed - as supplied by publisher]

Participation in a healthy lifestyle intervention such as the Diabetes Prevention Program Group Lifestyle Balance –adapted for stroke (GLB-CVA) may reduce stroke burden. Identifying biomarkers associated with lifestyle changes may enhance an individualized approach to stroke recovery. We investigated metabolic biomarkers related to cardiovascular and neurological function in individuals with stroke in the GLB -CVA study and healthy (non-stroke) individuals.

Type of admission is associated with outcome of spontaneous subarachnoid hemorrhage
ConclusionsSpontaneous subarachnoid hemorrhage patients admitted directly to our comprehensive stroke center showed less complications compared to those transferred from general hospitals. This improvement was independent of time to treatment. Additional research in multiple centers using prospective analysis should be conducted to confirm that preferential direct transport to a comprehensive stroke center would likely yield considerable improvements in public health.

Identification and Management of Ischemic Stroke in the Postanesthesia Care Unit
This report details the case of a 63-year-old male experiencing a stroke in the immediate postoperative period after total knee arthroplasty. Risk for perioperative stroke is influenced by age, sex, ethnicity, comorbidities, and some medications. The depressed neurocognitive state of patients recovering from anesthesia warrants special consideration for the identification and management of perioperative stroke.

Tenecteplase use in the management of acute ischemic stroke: Literature review and clinical considerations
CONCLUSION: Current clinical evidence shows that tenecteplase is not inferior to alteplase for the treatment of ischemic stroke and suggests that tenecteplase may have a superior safety profile. Furthermore, tenecteplase also has practical advantages in terms of its administration. In patients with cryptogenic stroke, long-term monitoring is standard-of-care to exclude atrial tachyarrhythmias. Prior studies have not examined interatrial block (IAB) and other ECG markers paired with long-term continuous monitoring (the current gold standard) to predict likelihood of atrial fibrillation (AF) detection in stroke patients.
