Gastrointestinal Bleeding in Acute Ischemic Stroke: Recent Trend and Predictors. (P6.040)
CONCLUSIONS:GIB is relatively known complication in setting of AIS. Decremental trend in recent years does not have any association with chemical or mechanical thrombolysis but instead was predicted by chronic altered metabolic states like NSAID use, alcohol abuse and Atrial fibrillation.Disclosure: Dr. Shah has nothing to disclose. Household Secondhand Smoke Exposure Confers Higher Stroke Risk and Poorer Stroke Prognosis (S42.005)
CONCLUSIONS: Household SHS is associated with a 1.5-fold increase in stroke risk and a 2-fold increase in mortality after stroke among never-smokers. While prospective studies are needed to assess causality, this study highlights the importance of obtaining SHS exposure history and counseling patients and their families on the potential impact of SHS on post-stroke outcomes.Disclosure: Dr. Lin has nothing to disclose. IV thrombolysis in patients with ischemic stroke and alcohol abuse
Conclusions: IS patients with chronic alcohol consumption or acute intoxication have decreased likelihood of receiving IVT and are not at an increased risk of associated SICH. This supports current practice guidelines, which do not list chronic alcohol consumption or acute intoxication as an exclusion criterion.

Clinical Reasoning: A 58-year-old man with right facial numbness
A 58-year-old man with a history of uncontrolled hypertension presented to the emergency department for evaluation of right facial numbness. He went to bed feeling normal and woke up with numbness and tingling in the right face limited to the right perioral area. He denied any associated neurologic symptoms or having similar experiences in the past. His medical history was significant for uncontrolled hypertension; surgical history was negative. Social history was pertinent for occasional alcohol use, but no smoking or drug use. Family history was significant for hypertension in both parents and essential tremor in his sis...

Watch your neighbor's garden, or Delphi's oracle for unruptured intracranial aneurysm treatment
Current management recommendations for patients with unruptured intracranial aneurysms (UIAs) include surgical clipping, endovascular coiling, or noninterventional follow-up.1 Several population-based studies and meta-analyses identified risk factors associated with aneurysm growth and rupture,2 some of which are modifiable (arterial hypertension, smoking, and excessive alcohol consumption) and some of which are not (increasing age, female sex, ethnic origin, aneurysm size, and morphology); however, no randomized controlled data exist on the potential benefit of preventive intervention for UIAs. The multinational TEAM (Tri...

An Unusual Cause Of Recurrent Ischemic Stroke: Trousseau's Syndrome From Gastric Cancer (P4.231)
CONCLUSIONS: Trousseau’s Syndrome must be considered in those presenting with recurrent strokes with an otherwise negative embolic work up especially in the elderly and those with cancer risk factors. It must prompt the clinician to look for an associated malignancy.Disclosure: Dr. JADEJA has nothing to disclose. Dr. Johnson has nothing to disclose. Dr. Soetanto has nothing to disclose. Seizure as the Presenting Symptom of ICH Patient Characteristics and EEG Utilization (P3.087)
Conclusions: Patients who presented with seizure were younger, black, and a higher proportion had a history of alcohol and substance abuse compared to patients with ICH who did not present with a seizure. Only 66[percnt] of those presenting with clinical seizure underwent EEG. Despite the prevalence of subclinical seizures in ICH patients, only 19[percnt] of patients who did not present with a seizure underwent EEG. Our study suggests that there may be room for improvement on the part of stroke neurologists in the diagnosis and management seizure of ICH patients.Disclosure: Dr. Gadpaille has nothing to disclose. The Longitudinal Relationship between Inflammatory Burden and Gait Speed is Influenced by White Matter Microstructural Characteristics: Secondary Analysis of the Health ABC Study. (I10-2A)
CONCLUSIONS: Sustained exposure to high IL-6 rather than the rate of change in IL-6 may adversely affect walking speed, potentially by influencing WM microstructural characteristics in older adults. Bilateral ascending weakness in a patient with Azygous Anterior Cerebral Artery Stroke. (P1.040)
CONCLUSION: Our patient’s clinical presentation was challenging because ascending pattern of weakness is usually seen in Acute Inflammatory Demyleinating Neuropathy or spinal cord injury. During the early stages deep tendon reflexes may be perserved in many conditions but may change on serial clinical examination. Hyperreflexia noted on follow-up examination and the absence of sensory involvemnt were the key feature suggesting a cerebral insult. Our study calls to attention to this rare vascular anomaly and its misleading clinical presentation.Disclosure: Dr. Melkumova has nothing to disclose. Dr. Noroozi has nothing...
