The impact of associated diabetic retinopathy on stroke and severe bleeding risk in diabetic patients with atrial fibrillation: The Loire Valley Atrial Fibrillation Project.
ConclusionCrude rates of stroke/TE increased in a stepwise fashion when non-diabetic AF patients were compared to diabetics with no retinopathy and diabetics with retinopathy. However, we have shown for the first time that the presence of diabetic retinopathy did not emerge as an independent predictor for stroke/TE or severe bleeding on multivariate analysis. Background: Diabetes mellitus is recognized as a stroke risk factor in atrial fibrillation (AF). Diabetics with retinopathy have an increased risk of systemic cardiovascular complications, and also, severe diabetic retinopathy predisposes to ocular bleeds. We hyp...

Oral anticoagulation and the risk of stroke or death in patients with atrial fibrillation and one additional stroke risk factor: the Loire Valley Atrial Fibrillation Project.
Conclusion: Among AF patients with one single additional stroke risk factor (CHA2DS2-VASc score = 1 in males, 2 in females), OAC use was associated with an improved prognosis for stroke/systemic thromboembolism/death. PMID: 26425935 [PubMed - as supplied by publisher]

Development of a novel composite stroke and bleeding risk score in patients with atrial fibrillation: The AMADEUS study.
CONCLUSION We have developed and validated 2 novel composite scores for stroke/thromboembolism/ bleeding that offer good discriminatory and predictive performance. However, these composite risk scores did not perform better than the easier and more practical 'traditional' stroke and bleeding risk scores that are currently in use, which allow greater practically and more personalised balancing of risks. Poor enhancement pattern of left atrial appendage in cardiac computed tomography is associated with stroke in persistent atrial fibrillation patients.
Conclusions: The LAA enhancement pattern observed using cardiac CT is associated with the risk of stroke. Poor enhancement in the LAA suggests a potential high risk of stroke. PMID: 31559034 [PubMed]

Relationship of the SAME-TT2R2score to poor quality anticoagulation, stroke, clinically relevant bleeding and mortality in patients with atrial fibrillation.
CONCLUSION We demonstrate that the SAME-TT2R2 score was predictive for an increasing risk of stroke/TE, severe bleeding, major BARC bleeding and death, reflecting poor anticoagulation control (and labile INRs) on VKA amongst our patients with AF. PMID: 24722973 [PubMed - as supplied by publisher]

Patent foramen ovale and stroke in intermediate-risk pulmonary embolism.
CONCLUSIONS: PFO and related ischemic strokes are frequent in intermediate-risk pulmonary embolism. TEE is much more efficient than TTE for PFO diagnostic. Considering the high risk of intra-cranial bleeding with thrombolysis in PE, which in part may be due to haemorrhagic transformation of sub-clinical strokes, screening PFO with TEE should be considered in intermediate risk PE when a thrombolytic treatment is discussed. Stroke, Major Bleeding and Mortality Outcomes in Warfarin Users with Atrial Fibrillation and Chronic Kidney Disease: A Meta-analysis of Observational Studies.
Conclusions: Based on this meta-analysis, the use of warfarin for AF may have an unfavorable risk/benefit ratio in patients with end-stage CKD but not in those with non-end stage CKD. PMID: 26378611 [PubMed - as supplied by publisher]

Pacemaker-detected atrial fibrillation burden and risk of ischemic stroke or thromboembolic events—A cohort study
ConclusionIn patients with dual-chamber pacemakers and comparable CHA2DS2-VASc score, pacemaker-detected AF burden is associated with elevated risk for thromboembolism. Further research is needed to clarify how pacemaker-detected AF burden could incorporate with CHA2DS2-VASc score variables and help to guide anticoagulation.

Aortic thrombosis: the forgotten source of ischemic stroke. A case report and systematic review of the literature
Monaldi Arch Chest Dis. 2021 Dec 3. doi: 10.4081/monaldi.2021.2090. Online ahead of print.ABSTRACTIschemic stroke represents one of the most important health problems in industrialized countries, both for epidemiological and socio-economic impact. The presence of thrombi in the aorta is rare and its treatment has not been uniquely defined. Here we report the case of an 82-years-old man with aortic thrombosis and acute ischemic stroke.PMID:34865461 | DOI:10.4081/monaldi.2021.2090

Stroke/thromboembolism and intracranial hemorrhage in a 'real world' atrial fibrillation population: The CAFBO STUDY (Complications of Atrial Fibrillation in the BOlogna area).
CONCLUSION IEs occurred mainly in elderly patients who received ASA or no treatment. Half of anticoagulated patients with IEs had subtherapeutic INRs. Therapeutic approaches to elderly subjects with AF require an effective anticoagulant treatment strategy. PMID: 24810397 [PubMed - as supplied by publisher]

Antithrombotic Therapy for Atrial Fibrillation: CHEST Guideline and Expert Panel Report.
CONCLUSIONS: Oral anticoagulation is the optimal choice of antithrombotic therapy for patients with AF with ≥1 non-gender CHA2DS2VASc stroke risk factor(s). PMID: 30144419 [PubMed - as supplied by publisher]

Long-term impact of newly diagnosed atrial fibrillation during critical care: A Korean nationwide cohort study.
CONCLUSION: The patients who survived after newly diagnosed AF during critical care remained at a higher risk of long-term stroke/SE and mortality than the patients without AF regardless of AF recurrence and the causes of critical care. Close follow-up and continuous anticoagulation might be needed for these patients. PMID: 31051171 [PubMed - as supplied by publisher]

Comparing the Effectiveness and Safety of Non-Vitamin K Antagonist Oral Anticoagulants and Warfarin in the Elderly Asian Patients with Atrial Fibrillation: A Nationwide Cohort Study.
CONCLUSIONS: Compared to warfarin, NOACs were associated with a significantly lower risk of adverse events, with heterogeneity in treatment effects among different age strata. Overall, the clear safety signal in favor of NOACs over warfarin was evident irrespective of age strata, being most marked in the most elderly. PMID: 31809694 [PubMed - as supplied by publisher]

The impact of smoking on thromboembolism and mortality in patients with incident atrial fibrillation: insights from the Danish Diet, Cancer and Health study.
CONCLUSIONS Smoking is associated with a higher risk of thromboembolism or death in patients with AF even after adjusting for well-recognised risk factors used in stroke risk stratification schemes. The associations may be modified by sex, as the associations were strongest among women. PMID: 24091709 [PubMed - as supplied by publisher]
