Is Endovascular Therapy for Stroke Cost-Effective Globally? A Systematic Review of the Literature
The standard of care for eligible patients with acute ischemic stroke (AIS) has been thrombolytic therapy with intravenous (IV) tissue plasminogen activator (tPA) within 4.5  h of symptom onset to achieve reperfusion and reduce morbidity and mortality.1-3 However, the emergence of endovascular therapy (EVT) has changed the treatment paradigm for large vessel occlusion (LVO) acute ischemic stroke (AIS). The evidence supporting the use of EVT as first-line treatment for LVO stroke is unequivocal.45

Comments on Coronavirus Positive Patients Presenting with Stroke-Like Symptoms
We read with interest the short communication entitled “Coronavirus Positive Patients Presenting with Stroke-Like Symptoms”,1 it was a case series on stroke presentation over COVID-19 patients. We are particularly interested on patient 10, the one and only one central retinal artery occlusion (CRAO) case secondary to occlusive thrombus in the intern al carotid artery (ICA), who was also highlighted by the authors. CRAO is a blinding ocular emergency, and a harbinger of ischemic cerebrovascular stroke.

Infectious Causes of Stroke
Stroke is one of the primary causes of mortality and morbidity worldwide. It can be ischemic or hemorrhagic, and the former can be due to an in-situ thrombus or a distant embolus. Despite being a rare cause, stroke can also be caused in the setting of infection. Bacterial agents are the most common cause of stroke, among other infectious agents. Until the antibiotic era, rheumatic heart disease was a predisposing risk factor of infective endocarditis. VZV is the most common cause of strokes in pediatric and adult populations.

Endovascular Treatment for Acute Ischemic Stroke After Video-Assisted Thoracoscopic Pulmonary Lobectomy in Patients With Lung Cancer: A Report of Four Cases and Literature Review
Postoperative cerebral embolic stroke is a serious complication of pulmonary lobectomy, occurring in 1.1% of patients undergoing lobectomy through video-assisted thoracoscopic surgery (VATS). The mechanism of this complication is thought to be embolic stroke caused by thrombus formed due to stagnation in the pulmonary vein stump after VATS lobectomy. There have been few reports demonstrating the utility of endovascular treatment (EVT) for cerebral embolic stroke after VATS lobectomy.

Moving from traditional to more advanced treatments in stroke care is cost-effective: A case study from Greece
Ischemic stroke (IS) is one of the leading causes of mortality and disability,1, 2 with the economic burden of post-stroke care being substantial.1, 2 After incidence of IS, the following effective treatments have been found to significantly improve survival and quality of life: (a) treatment in specialized centres (Stroke Units; SU), (b) use of intravenous thrombolytic therapy (IVT), (c) use of endovascular thrombectomy (EVT) for patients with large vessel occlusions (LVO) and (d) rehabilitation.

Ischemic stroke is a frequently occurring cerebrovascular disease (CVD) linked to high mortality, disability, and incidence rates, and the second factor causing global mortality41,3. There are two common sources of embolism in ischamic strokes: large artery atherosclerosis and small artery atherosclerosis. Ischemic stroke can be treated clinically with thrombolytic therapy4. Although ischemic stroke is treatable, post-cerebral ischemia blood reperfusion frequently induces inflammation, oxidative stress (OS) or additional unfavorable events that may exacerbate brain injury20,24.

The Effects of Alteplase 3 to 6 Hours After Stroke in the EPITHET-DEFUSE Combined Dataset: Post Hoc Case-Control Study Original Contributions
Conclusions— The data provide further evidence that alteplase significantly attenuates infarct growth and increases reperfusion compared with placebo in the 3- to 6- hour time window in patients selected based on MR penumbral imaging. Density of Thrombus on Admission CT Predicts Revascularization Efficacy in Large Vessel Occlusion Acute Ischemic Stroke Brief Reports
Conclusions— Thrombi with lower HU on NECT appear to be more resistant to pharmacological lysis and mechanical thrombectomy. Measuring thrombus density on admission NECT provides a rapid method to analyze clot composition, a potentially useful discriminator in selecting the most appropriate reperfusion strategy for an individual patient.

Body temperature, blood infection parameters, and outcome of thrombolysis‐treated ischemic stroke patients
ConclusionA lower level of systemic inflammation at time of thrombolysis may be associated with clinical improvement and good outcome at three‐months. Increase in body temperature during the first 24 h associates with lack of clinical improvement and worse patient outcome.
