Case of Paradoxical Embolic Transient Ischemic Attack from an Unusual Extra Cardiac Shunt Detected on a Contrast‐Enhanced Echocardiogram
We describe a case of 63‐year‐old female who presented with a transient ischemic attack and at agitated saline contrast echocardiogram was found to have an unusual type of an extracardiac RLS. Further evaluation leads to diagnosis of superior vena cava (SVC) thrombosis from a prior indwelling central venous catheter. The SVC thrombosis induced systemic‐pulmonary venous collaterals were responsible for this unusual extracardiac RLS and paradoxical transient ischemic attack. We discuss the diagnosis and management of this rare clinical case.

Systemic treatments for the prevention of venous thrombo-embolic events in paediatric cancer patients with tunnelled central venous catheters.
CONCLUSIONS: We found no significant effects of systemic treatments compared with no intervention in preventing (a)symptomatic VTE in paediatric oncology patients with CVCs. However, this could be a result of the low number of included participants, which resulted in low power. In one CCT, which compared one systemic treatment with another systemic treatment, we identified a significant reduction in symptomatic VTE with the addition of LMWH to AT supplementation. All studies investigated the prevalence of major and/or minor bleeding episodes, and none found a significant difference between study groups. None of the studies ...

Arterial Closure Devices for Treatment of Inadvertent Large-Caliber Catheter Insertion Into the Subclavian or Carotid Artery: A Case Series of Five Patients
CENTRAL VENOUS CATHETERIZATION is a common procedure in today`s medicine. Iatrogenic complications occur in up to 34% of catheter placements including 5% of arterial puncture. A variety of complications by arterial puncture have recently been described, including airway obstruction, pseudoaneurysms, arteriovenous fistulas, and even stroke caused by arterial thrombosis or embolization. Although 2-D ultrasound assisted insertion of large central venous catheters may reduce adverse events, it might not be available in an emergency setting, and the use of ultrasound guidance requires training. Depending on anatomic localizatio...
