Genomic Observations of a Rare/Pathogenic SMAD3 Variant in Loeys ⁻Dietz Syndrome 3 Confirmed by Protein Informatics and Structural Investigations.
Conclusion: This case emphasizes the importance of comprehensive genetic testing to evaluate patients for connective tissue disorders, as well as the potential benefit of utilizing a protein informatics platform for the assessment of variant pathogenicity. PMID: 31096651 [PubMed - in process]

The Addition of Aortic Root Procedures during Elective Arch Surgery Does Not Confer Added Morbidity nor Mortality.
CONCLUSIONS: The addition of an aortic root procedure during elective aortic arch surgery lengthens cardiopulmonary bypass and aortic crossclamp times but does not increase postoperative morbidity nor mortality. PMID: 30851260 [PubMed - as supplied by publisher]

Aortic valve replacement by mechanical prosthesis in children: A positive long-term outcome including for patients with an infantile Marfan syndrome
ConclusionsLong-term results after AVR are excellent. Complications associated with surgery and long-term anticoagulation are rare. This study gives confidence in aortic mechanical prosthesis valve replacement including for children with an infantile MS.

Long-Term Results of Aortic Root Surgery in Marfan Syndrome Patients: A Single-Center Experience.
CONCLUSIONS: The Bentall and David operations are both associated with satisfactory long-term results in MFS patients. The low rate of valve prosthesis-related complications suggested that the Bentall operation would continue to be a standard surgical treatment. The reimplantation technique, adopted for less-dilated aortas, provides satisfactory freedom from reoperation. Careful attention should be paid to the reimplantation technique in patients affected by a serious familiar genetic phenotype. PMID: 29302938 [PubMed - in process]

Elective David I Procedure Has Excellent Long-Term Results: 20-Year Single-Center Experience
Conclusions The valve-sparing David I procedure has excellent short- and long-term results. Erosion due to supposed leaflet contact with the straight tube graft was not observed in any patient, proving that using a straight graft has no negative impact on the leaflets. The “spared valve” (being native living tissue) seems to be more resistant to infection than prosthetic valves.

Increased Prevalence of Cerebrovascular Disease in Hospitalized Patients with Marfan Syndrome
Small studies have suggested that Marfan syndrome is associated with a number of cerebrovascular complications. We sought to determine whether a clinical diagnosis of Marfan syndrome is associated with a higher prevalence of cerebrovascular diseases than the general population by performing a case-control study of hospitalized patients in the Nationwide Inpatient Sample (NIS).

Florida Sleeve Procedure Is Durable and Improves Aortic Valve Function in Marfan Syndrome Patients.
CONCLUSIONS: The FS procedure can be performed safely in Marfan syndrome patients with immediate improvement in aortic valve function. Long-term survival and freedom from reoperation rates are encouraging. PMID: 28410640 [PubMed - indexed for MEDLINE]

Rethinking the Paradigm: Modern Approach to Proximal Aortic Reconstruction Demonstrates Excellent Outcomes.
CONCLUSION: Elective surgery for aneurysm of the proximal aorta is safe, reproducible, and is associated with outcomes that are superior to those seen in an acute aortic syndrome. It may be appropriate to offer surgery to younger patients with proximal aortic aneurysms at smaller diameters, even if their aortic dimensions do not yet meet traditional guidelines for surgical intervention. Endovascular thrombectomy in the setting of aortic dissection
Conclusions These three cases suggest that endovascular thrombectomy is feasible and can be done safely and efficaciously in patients with aortic dissections and those with Marfan syndrome, although the risks and benefits should be considered as part of any decision-making process. Given that endovascular therapy for acute stroke is now in many situations part of standard care, further studies will be necessary to delineate more precise inclusion and exclusion criteria.

Clinical Outcomes of the David V Valve-Sparing Root Replacement Compared With Bioprosthetic Valve-Conduits for Aortic Root Aneurysms
Conclusions VSRR and BIO result in equivalent operative mortality and morbidity with similar midterm survival and valve durability. VSRR is an effective alternative to BIO for aortic root pathologic conditions; however, careful patient selection is paramount.

The effect of losartan therapy on ventricular function in Marfan patients with haploinsufficient or dominant negative FBN1 mutations.
CONCLUSION: Losartan therapy in haploinsufficient Marfan patients increases biventricular end diastolic volume and stroke volume, furthermore, losartan also appears to ameliorate biventricular filling properties. PMID: 27704402 [PubMed - as supplied by publisher]
