Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes syndrome: a case report from Nepal
Ann Med Surg (Lond). 2023 May 3;85(6):3026-3030. doi: 10.1097/MS9.0000000000000712. eCollection 2023 Jun.ABSTRACTMitochondrial encephalomyopathy, lactic acidosis, and stroke-like syndrome (MELAS) is a rare neurodegenerative inherited disorder that is characterized by stroke-like episodes, seizures, endocrine, and multiple system involvement. It is important to consider it as a differential diagnosis in a young patient with stroke-like episodes as it is progressive and has multiple complications.CASE PRESENTATION: A 28-year-old male presented with slurring of speech and drowsiness for 7 h. He was a diagnosed case of type 2 ...

'Patient unable to express why he was on the floor, he has aphasia.' A content thematic analysis of medical records and incident reports on the falls of hospital patients with communication disability following stroke
CONCLUSIONS & IMPLICATIONS: The medical records and incident reports of patients with communication disability following stroke reveal that hospital staff recognize the impact of communication disability as potential risk factors for falls for this group. It was difficult for staff to report the circumstances of the fall for patients with severe communication disability. Despite the recognition of communication as a potential contributing factor, few medical record entries documented strategies related to communication interventions to improve patients' ability to understand instructions, gain attention or communicate basi...

Turning the Spotlight to Cholinergic Pharmacotherapy of the Human Language System
AbstractEven though language is essential in human communication, research on pharmacological therapies for language deficits in highly prevalent neurodegenerative and vascular brain diseases has received little attention. Emerging scientific evidence suggests that disruption of the cholinergic system may play an essential role in language deficits associated with Alzheimer ’s disease and vascular cognitive impairment, including post-stroke aphasia. Therefore, current models of cognitive processing are beginning to appraise the implications of the brain modulator acetylcholine in human language functions. Future work sho...

Intersectional sociodemographic and neurological relationships in the naming ability of persons with post-stroke aphasia
CONCLUSIONS: The findings reported here suggest higher income and larger family size are associated with better outcomes. As expected, aphasia type was significantly associated with naming outcomes. However, poorer performance by Black PWA and individuals with low income suggests that SDOH can play a critical role (positive and negative) in naming impairment in some populations with aphasia.PMID:37331326 | DOI:10.1016/j.jcomdis.2023.106352

When Task is Huge but Time is Short! An Interesting Case of Stroke Thrombolysis
Debabrata Chakraborty, Suvadip Maiti, Sadanand Dey, Sanjay BhaumikNeurology India 2023 71(3):552-554 A 64-year-old gentleman, diabetic, and smoker attended the emergency room (ER) with acute stroke. He had expressive aphasia with right upper limb weakness. His blood pressure was extremely high, and he presented in the final half an hour of the permissible window period for thrombolysis. Bringing down his blood pressure to make him eligible for the procedure within the available time was a real challenge for us. Fortunately, we succeeded in our attempt and he gradually improved. Truly, the maximum blood pressure allowable ...

'It's been an extraordinary journey': Experience of engagement from the perspectives of people with post-stroke aphasia
CONCLUSIONS & IMPLICATIONS: Engagement is a dynamic, multifaceted and person-centred process involving the client, provider and rehabilitation context. Findings from this work have implications for measuring engagement, training student clinicians to be skilled facilitators in engaging their clients and implementing person-centred practices that promote engagement within clinical settings.WHAT THIS PAPER ADDS: What is already known on the subject Engagement is recognized as an important factor in rehabilitation treatment response and outcomes. Prior literature suggests that the therapist plays a critical role in facilitati...
