Single or dual antiplatelet therapy after transcatheter aortic valve replacement: an updated systemic review and meta-analysis.
Conclusions: This meta-analysis suggests that compared with DAPT, SAPT after TAVR is more likely to lead to a decline of 30-day mortality along with the reduced risk of bleeding and no increased risk of stroke. However, more clinical data and evidence from randomized controlled trials are warranted to clarify the optimal post-TAVR antiplatelet strategy. PMID: 31019786 [PubMed]

Aspirin use for primary prevention in elderly patients.
Authors: Terrosu P Abstract The net clinical benefit of aspirin in primary prevention is uncertain as the reduction in occlusive events needs to be balanced against the increase in gastro-intestinal and cerebral bleedings. The meta-analysis of ATT (Anti Thrombotic Trialists) Collaboration in 2009 showed that aspirin therapy in primary prevention was associated with 12% reduction in cardio-vascular events, due mainly to a reduction in non-fatal myocardial infarction (0.18% vs 0.23% per year, p<0.0001). Time trends of aspirin and warfarin use on stroke and bleeding events in Chinese patients with new-onset atrial fibrillation.
Conclusion: Aspirin use increased amongst newly diagnosed Chinese AF patients with no relationship to the patient's stroke or bleeding risk. Warfarin use was very low. Given the healthcare burden of AF and its complications, our study has major implications for healthcare systems in non-Western countries, given the global burden of this common arrhythmia. Development of a novel composite stroke and bleeding risk score in patients with atrial fibrillation: The AMADEUS study.
CONCLUSION We have developed and validated 2 novel composite scores for stroke/thromboembolism/ bleeding that offer good discriminatory and predictive performance. However, these composite risk scores did not perform better than the easier and more practical 'traditional' stroke and bleeding risk scores that are currently in use, which allow greater practically and more personalised balancing of risks.
