Patient-oriented composite endpoints and net adverse clinical events with ticagrelor monotherapy following percutaneous coronary intervention: Insights from the randomized GLOBAL LEADERS trial.
CONCLUSIONS: The experimental treatment strategy of one-month DAPT followed by 23 months of ticagrelor alone did not result in a significant reduction in the rates of site-reported POCE or NACE, when compared to the reference treatment. PMID: 30888959 [PubMed - as supplied by publisher]

Optimizing Patient Discharge Management after Transfemoral Transcatheter Aortic Valve Implantation: The Multicentre European FAST-TAVI Trial.
CONCLUSIONS: We validated the appropriateness of a pre-specified set of risk criteria that allow for a safe and timely discharge. The rate of 30-day complications did not reveal any risk increase with this strategy compared with the reported outcomes in major TAVI trials and registries. PMID: 30777842 [PubMed - as supplied by publisher]

Early commercial experience from transcatheter aortic valve implantation using the Portico ™ bioprosthetic valve: 30-day outcomes in the multicentre PORTICO-1 study.
CONCLUSIONS: The Portico TAVI system allows safe and effective treatment of aortic stenosis in patients at increased surgical risk. At 30 days, mortality was low, and good haemodynamic performance was indicated by low transvalvular gradient and a low rate of moderate or higher PVL. PMID: 30111522 [PubMed - as supplied by publisher]

Radial versus femoral artery access in patients undergoing PCI for left main coronary artery disease: analysis from the EXCEL trial.
CONCLUSIONS: In the EXCEL trial, PCI of LM disease with TRA was associated with comparable early and late clinical outcomes as TFA. PMID: 30111521 [PubMed - as supplied by publisher]

Randomized Evaluation of a Novel Biodegradable Polymer-Based Sirolimus-Eluting Stent in ST-Segment Elevation Myocardial Infarction: The MASTER Study.
CONCLUSIONS: The study showed clinical non-inferiority and angiographic superiority of BP-SES versus a comparator BMS, suggesting that this novel DES may be a potential treatment option in STEMI. PMID: 29957593 [PubMed - as supplied by publisher]

Long-term effectiveness and safety of transcatheter closure of patent foramen ovale compared with antithrombotic therapy: A meta-analysis of 6 randomised clinical trials and 3560 patients with reconstructed time-to-event data.
CONCLUSIONS: Compared with ATA, tPFOc reduces the risk of stroke at long-term follow-up but no benefit is observed in terms of TIA. Atrial fibrillation is higher after tPFOc, while major bleeding and migraine are comparable between groups. PMID: 29901447 [PubMed - as supplied by publisher]

Transcatheter aortic valve implantation with the 34 mm self-expanding CoreValve Evolut R: initial experience in 101 patients from a multicenter registry.
CONCLUSIONS: Initial experience with the new self-expanding CoreValve Evolut R® 34 mm valve is characterized by high procedural success, excellent hemodynamic performance and a low early complication rate. PMID: 29808820 [PubMed - as supplied by publisher]

Left atrial appendage occlusion with the AMPLATZER ™ Amulet™ device: one-year follow-up from the prospective global Amulet Registry.
CONCLUSIONS: In the global prospective Amulet Registry of patients at high-risk of stroke and bleeding, the annualised ischaemic stroke rate was 2.9%. The LAA was sealed in 98.4% after 1-3 months and device related thrombus was observed in 1.7% of cases with only a minority of all patients on anticoagulation treatment. PMID: 29806820 [PubMed - as supplied by publisher]

Impact of Sex on Comparative Outcomes of Bivalirudin versus Unfractionated Heparin in Patients with Acute Coronary Syndromes Undergoing Invasive Management A pre-specified analysis of the MATRIX trial.
CONCLUSIONS: In ACS patients, the rates of MACE and NACE were not significantly lower with bivalirudin than with UFH in both sexes. The rate of the composite of urgent TVR,definite ST,or NACE was not significantly lower with a post-PCI bivalirudin infusion than with no post-PCI infusion in both sexes. Short-term dual antiplatelet therapy after interventional left atrial appendage closure with different devices.
CONCLUSIONS: Short-term DAPT for six weeks appears to be a viable alternative for patients after LAAC. Age ≥75 years and renal impairment increase major bleeding events threefold. PMID: 29400653 [PubMed - as supplied by publisher]

Real world safety and efficacy of WATCHMAN LAA closure at one year in patients on dual antiplatelet therapy Results of the DAPT subgroup from the EWOLUTION all-comers study.
CONCLUSIONS: LAA closure with the WATCHMAN followed by DAPT therapy in a high risk patient population is safe. At one year the intervention is associated with a substantial risk reduction regarding ischemic stroke and major bleeding compared to the expected rate based on CHA2DS2-VASc and HAS-BLED scores. Clopidogrel or ticagrelor in acute coronary syndrome patients treated with newer-generation drug-eluting stents: CHANGE DAPT.
CONCLUSIONS: In this observational study, the guideline-recommended ticagrelor-based primary DAPT regimen was associated with an increased event risk in consecutive ACS patients treated with newer-generation DES. PMID: 29151439 [PubMed - in process]

Transcatheter Left Atrial Appendage Occlusion for Stroke Prevention in Patients with Atrial Fibrillation : Results from the Belgian Registry.
CONCLUSIONS: The data of the Belgian left atrial appendage occlusion registry suggest that the procedure is effective and relatively safe in a real world setting, using either the Watchman or the ACP/Amulet device. PMID: 28966159 [PubMed - as supplied by publisher]

Left atrial appendage occlusion.
Authors: Nietlispach F, Moarof I, Taramasso M, Maisano F, Meier B Abstract Left atrial appendage occlusion (LAAO) is a first-line therapy for stroke prevention in patients suffering from atrial fibrillation (AF). While patients at high risk for bleeding or patients with previous bleeding have the highest relative risk reduction from LAAO, it is the younger patients with a high lifetime bleeding risk who benefit from the highest absolute risk reduction after LAAO. Therefore, LAAO should be discussed with every AF patient, as an alternative to lifelong oral anticoagulation. The patient needs to be informed about the ...

Prevalence and Impact of Critical Limb Ischemia on In-Hospital Outcome in Transcatheter Aortic Valve Implantation in Germany.
CONCLUSIONS: In patients undergoing TAVI, presence of PAD is associated with an increased risk of periprocedural complications, while only CLI predicts independently increased in-hospital mortality. Whether CLI represents a marker of general poor health status resulting in the poor outcome or a modifiable risk factor whose treatment prior to TAVI can improve the outcome requires prospective studies.
