Simvastatin, but Not Atorvastatin, Is Associated with Higher Peak Rivaroxaban Serum Levels and Bleeding: an Asian Cohort Study from Singapore
ConclusionOur findings suggest that concomitant use of simvastatin with rivaroxaban may be associated with bleeding events in an Asian cohort. Further studies using physiologically based pharmacokinetic modelling are required to investigate the drug-drug interactions between these drugs.

Electrocardiogram alterations in non-traumatic brain injury: a systematic review
AbstractThe presence of abnormal electrocardiograms in individuals without known organic heart disease is one of the most common manifestations of cardiac dysfunction occurring during acute non traumatic brain injury. The primary goal of the present review is to provide an overview of the available data and literature regarding the presence of new-onset electrocardiographic (ECG) alterations in acute non traumatic brain injury. The secondary aim is to identify the incidence of ECG alterations and consider the prognostic significance of new-onset ECG changes in this setting. To do so, English language articles from January ...

De-escalation from ticagrelor to clopidogrel in patients with acute myocardial infarction: the TALOS-AMI HBR substudy
CONCLUSIONS: In stabilised acute myocardial infarction patients, unguided de-escalation from ticagrelor to clopidogrel was associated with a lower rate of net adverse clinical outcomes irrespective of HBR status. The effect of de-escalation of P2Y12 inhibition on reducing haemorrhagic events was greater in patients with HBR.PMID:37724337 | DOI:10.4244/EIJ-D-23-00427

Polypharmacy and Bleeding Outcomes After Percutaneous Coronary Intervention
CONCLUSIONS: In a real-world population of patients undergoing PCI, approximately 90% were on ≥5 medications. Increasing number of medications was associated with a higher adjusted risk for major bleeding, but not ischemic events.PMID:37722886 | DOI:10.1253/circj.CJ-23-0558

Another Step Forward in Our Ability to Predict Percutaneous Coronary Intervention Outcomes
Despite advancements in devices, technique, and patient selection, adverse events including bleeding, vascular complications, myocardial infarction, stroke, and death still occur during percutaneous coronary intervention (PCI). Accurate and up-to-date risk-adjusted models that predict mortality after PCI are important to inform clinical decision making, identify patient factors that increase risk, provide a standardized format to measure programmatic quality, and drive continuous quality improvement efforts to improve operator outcomes.

ISCHEMIC STROKE AND MAJOR BLEEDING WHILE ON DIRECT ORAL ANTICOAGULANTS IN NA ÏVE PATIENTS WITH ATRIAL FIBRILLATION: IMPACT OF RESUMPTION OR DISCONTINUATION OF ANTICOAGULANT TREATMENT. A population-based study.
We assessed the cumulative incidence of recurrent stroke, major bleeding and all-cause mortality associated with restarting antithrombotic treatment, in patients experiencing an anticoagulation-related event (stroke or major bleeding), occurred during anticoagulation therapy for AF.Methods and Results. We performed a retrospective population-based analysis on linked claims data of patients resident in the Veneto Region, treated with DOACs for AF and discharged (2013 −2020) from the hospital for stroke, intracranial haemorrhage (ICH), and major bleeding.

Off-Label Reduced Dose Apixaban in Older Adults With Atrial Fibrillation and Associated Outcomes
CONCLUSION AND RELEVANCE: Older adults with nonvalvular AF are commonly prescribed lower-than-recommended doses of apixaban. However, no significant association was found between empiric off-label reduced dosing and stroke or bleeding outcomes.PMID:37712551 | DOI:10.1177/10600280231199137

Platelet glycoprotein IIb/IIIa inhibitor tirofiban in clopidogrel-na ïve patients undergoing elective percutaneous coronary intervention
CONCLUSION: Ad-hoc PCI in clopidogrel-naïve patients who were treated with high-dose bolus of tirofiban followed by a clopidogrel loading dose immediately after the procedure appeared to be safe.PMID:37713051 | DOI:10.1007/s12471-023-01810-2
