Retrospective Case-Control Study of REGEN-COV (casirivimab and imdevimab) Therapy for Patients with COVID-19 and Cancer Using the United States MarketScan ® Database
Conclusion Patients with cancer treated with REGEN-COV experienced a decreased risk for hospitalization, hospitalization duration, and total COVID-19-related costs. Patients with cancer were at a higher risk of being hospitalized for COVID-19 than were those without cancer. The use of neutralizing antibody therapy may reduce the risk of severe COVID-19 infection for patients with cancer with an otherwise high risk. Future replication studies should be conducted using other databases that include Medicaid users and other insured persons for comparison and validation.PMID:37666220 | DOI:10.1159/000533614

Development and Validation of a Claims-Based Model to Predict Levels of Obesity
Am J Epidemiol. 2023 Aug 30:kwad178. doi: 10.1093/aje/kwad178. Online ahead of print.ABSTRACTWe developed and validated a claims-based algorithm that classifies patients into obesity categories. Using Medicare (2007-2017) and Medicaid (2000-2014) claims data linked to two electronic health records (EHR) systems in Boston, Massachusetts, United States, we identified a cohort of patients with an EHR-based BMI measurement. We used regularized regression to select from 137 variables and built generalized linear models to classify patients with BMI≥25, BMI≥30 and BMI≥40. We developed the prediction model using EHR system-...

Diabetes and Hypertension Prevention and Control in Community Health Centers: Impact of the Affordable Care Act
J Prim Care Community Health. 2023 Jan-Dec;14:21501319231195697. doi: 10.1177/21501319231195697.ABSTRACTAccess to care significantly improved following the implementation of the Patient Protection and Affordable Care Act. Since its implementation, the number of uninsured Americans has significantly decreased. Medicaid expansion played an important role in community health centers, who serve historically marginalized populations, leading to increased clinic revenue, and improved access to care. As the continuous Medicaid enrollment provision established during the pandemic ended, and states have to make decisions about thei...

Autumn in New York: The case of long-term care facilities in the "Safe Staffing" lawsuit with less staffing but similar COVID-19 outcomes
DISCUSSION AND IMPLICATIONS: LTCFs involved in the lawsuit were deemed by policymakers as reducing staff, earning excess profits, and placing residents at-risk. While these LTCFs reported lower staffing levels, we observed no differences in resident outcomes during the first year of the COVID-19 pandemic. Researchers and policy makers should develop more nuanced perspectives concerning the relationship among LTCF staffing, outcomes, and organizational profitability.PMID:37638966 | DOI:10.1093/geront/gnad118

Disparities in Survival of Hematologic Malignancies in the Context of Social Determinants of Health: A Systematic Review
Blood Adv. 2023 Aug 28:bloodadvances.2023010690. doi: 10.1182/bloodadvances.2023010690. Online ahead of print.ABSTRACTSocial Determinants of Health (SDH) have been reported as relevant factors responsible for health inequity. We sought to assess clinical data from observational studies evaluating the impact of SDH on the outcomes of patients with hematologic malignancies. Thus, we performed a systematic review in 6 databases on September 1, 2021, in which paired reviewers independently screened studies and included data from 41 studies. We assessed the risk of bias by the Joanna Briggs Institute appraisal tools and analyze...

Trends in and Prognostic Significance of Time to Treatment in Pancreatic Cancer: A Population-Based Study
CONCLUSIONS: Delayed treatment approaching 2 months was observed in 10% of the population. The rising temporal trend in TTT may be attributed to the increasing shift toward neoadjuvant CT/RT in early-stage PDAC and/or the increasing use of tissue biopsy prior to surgery.PMID:37624518 | DOI:10.1245/s10434-023-14221-9

Inequities in referrals to a breast cancer risk assessment and prevention clinic: a mixed methods study
CONCLUSIONS: In this single-site evaluation, among individuals referred by primary care clinicians for specialized breast care, Black, publicly-insured patients, and those whose preferred language was not English were less likely to be referred for risk assessment. Larger studies are needed to confirm these findings. Interventions to standardize breast cancer risk assessment in primary care may improve equity.PMID:37626335 | DOI:10.1186/s12875-023-02126-1

Predicting hospital readmission in Medicaid patients with diabetes using administrative and claims data
CONCLUSIONS: Our model has moderate power for identifying hospitalized Medicaid patients with diabetes who are at high risk of readmission. It is a template for identifying patients at risk of readmission and for adjusting comparisons of 30-day readmission rates among sites or over time.PMID:37616150 | DOI:10.37765/ajmc.2023.89409

Pharmacist-led review of empagliflozin and ertugliflozin following formulary update
CONCLUSIONS: Following the HNJH Medicaid HMO's coverage update from empagliflozin to ertugliflozin, some patients received inappropriate therapy and providers accepted clinical pharmacists' recommendations to optimize therapy.PMID:37616149 | DOI:10.37765/ajmc.2023.89408
