Effectiveness and safety of opioids on breathlessness and exercise endurance in patients with chronic obstructive pulmonary disease: A systematic review and meta-analysis of randomised controlled trials
CONCLUSIONS: Sustained-release opioids did not improve dyspnoea and exercise endurance. Short-acting opioids appeared to be safe, have potential to lessen dyspnoea and improve exercise endurance, supporting benefit in managing episodes of breathlessness and providing prophylactic treatment for exertional dyspnoea.PMID:37710987 | DOI:10.1177/02692163231194838

Paediatric Palliative Care in a Reference Centre of Inherited Metabolic Diseases
CONCLUSION: Mitochondrial, lysosomal, and peroxisomal disorders are complex multisystemic diseases that very often have no treatment intended to cure. These patients have a heavy symptom burden and frequent intercurrences. Addressing these symptoms is challenging, but PPC has proven to reduce hospital admissions with consequent improvement in quality of life. Radiocarpal arthrodesis revision with a pyrocarbon implant
We report the case of a patient who was dissatisfied with the outcome of radiocarpal arthrodesis, due to lack of motion despite complete absence of pain. We replaced arthrodesis by a pyrocarbon interposition implant. At 20 months' follow-up, the results showed complete absence of pain, partially restored range of motion and the implant in good position on radiographs. We believe that pyrocarbon implants may be a solution for patients dissatisfied with palliative treatment by arthrodesis.PMID:37709254 | DOI:10.1016/j.hansur.2023.08.010

CONCLUSIONS: Though income is only one factor physicians consider in deciding where and how to work, it is concerning to see lower incomes among groups that traditionally experience shortages (eg, geriatrics and government-associated practice sites).
