Acute Respiratory Failure in Pediatric Hematopoietic Cell Transplantation: A Multicenter Study*
Objectives: Acute respiratory failure is common in pediatric hematopoietic cell transplant recipients and has a high mortality. However, respiratory prognostic markers have not been adequately evaluated for this population. Our objectives are to assess respiratory support strategies and indices of oxygenation and ventilation in pediatric allogeneic hematopoietic cell transplant patients receiving invasive mechanical ventilation and investigate how these strategies are associated with mortality. Design: Retrospective, multicenter investigation. Setting: Twelve U.S. pediatric centers. Patients: Pediatric allogenei...

Candidemia Diagnosis With T2 Nuclear Magnetic Resonance in a PICU: A New Approach
CONCLUSIONS: T2Candida might be useful for the diagnosis of candidemia in PICUs. The prevalence of candidemia might be underestimated in this pediatric population. The use of this diagnostic tool in these units may help clinicians to start adequate and timely antifungal treatments.

Delirium documentation in hospitalized pediatric patients with cancer
Palliat Support Care. 2021 Mar 1:1-4. doi: 10.1017/S1478951521000171. Online ahead of print.ABSTRACTOBJECTIVE: Screening tools for delirium are being used more consistently in pediatric critical care. However, screening is not universal, and delirium may be underdiagnosed, misdiagnosed, or undocumented in hospitalized patients. We evaluated the identification and documentation of delirium in pediatric oncology and bone marrow transplant patients.METHOD: A retrospective chart review on all hospitalized pediatric oncology and bone marrow transplant patients admitted to an Academic Cancer center between 2013 and 2016. Patient...

Cardiac Dysfunction Following Brain Death in Children: Prevalence, Normalization, and Transplantation
We examined an organ procurement organization database (Life Center Northwest) of potential pediatric cardiac donors diagnosed with brain death between January 2011 and November 2013. Intervention: Transthoracic echocardiograms were reviewed for cardiac dysfunction (defined as ejection fraction

Compelling Ethical Challenges in Critical Care and Emergency Medicine
This new book addresses the ethical problems that physicians have to face every day while caring for critically ill patients. Improved treatments in emergency and critical care, have resulted in more patients surviving with reduced quality of life. This entails trade-offs for many patients, their families, and the teams caring for them. In 16 chapters experts in their respective fields describe compelling ethical challenges resulting from these discrepancies and discuss potential solutions. How Ethics Can Support Clinicians Caring for Critically Ill Patients Patients and Teams Caring for Them: Parallels Between Critical Car...

Transcranial Doppler Ultrasonography in Children With Acute Liver Failure and Severe Hepatic Encephalopathy
OBJECTIVES: To report our single-center use of transcranial Doppler (TCD) for noninvasive neuromonitoring in pediatric patients with acute liver failure (ALF). DESIGN: Retrospective cohort from January 2016 to June 2019. SETTING: PICU in Bicêtre Hospital, Assistance Publique- Hôpitaux de Paris (AP-HP), a national referral center for pediatric liver transplantation. PATIENTS: Pediatric patients with severe ALF (prothrombin time

Continuous Veno-Venous Hemodiafiltration and Plasma Exchange in Infantile Acute Liver Failure
Objectives: The purpose of the current study was to assess our multidisciplinary approach consisting of early application of neurology-oriented intensive care, aggressive artificial liver support and liver transplantation at the appropriate time for infants with acute liver failure. Design: Retrospective cohort study. Setting: A tertiary pediatric medical center in Japan. Patients: Seventeen infants younger than 12 months with acute liver failure who subsequently underwent liver transplantation between February 2006 and June 2011. Interventions: None. Measurements and Main Results: The patients varied from 1 to 11 months, ...

Surgeon Perceptions of Volume Threshold and Essential Practices for Pediatric Thyroidectomy
The prospect of sub-specialization within pediatric surgery has become increasingly debated over the past 10 years. Particularly within large groups or practices, some pediatric surgeons are choosing to narrow their clinical focus toward a particular subset of pathologies [1]. Further, in the past few years, additional clinical fellowships have been developed to provide board certified pediatric surgeons concentrated training in pediatric colorectal surgery, fetal surgery, critical care, and pediatric transplant surgery [1].

Predictors of Mortality in Pediatric Patients on Venoarterial Extracorporeal Membrane Oxygenation*
Objectives: Currently, there are no established echocardiographic or hemodynamic predictors of mortality after weaning venoarterial extracorporeal membrane oxygenation in children. We wished to determine which measurements predict mortality. Design: Over 3 years, we prospectively assessed six echo and six hemodynamic variables at 3–5 circuit rates while weaning extracorporeal membrane oxygenation flow. Hemodynamic measurements were heart rate, inotropic score, arteriovenous oxygen difference, pulse pressure, oxygenation index, and lactate. Echo variables included shortening/ejection fraction, outflow tract Doppler-derive...

Risk Factors of Acute Kidney Injury in Critically Ill Children*
Objectives: Acute kidney injury may be promoted by critical illness, preexisting medical conditions, and treatments received both before and during ICU admission. We aimed to estimate the frequency of acute kidney injury during ICU treatment and to determine factors, occurring both before and during the ICU stay, associated with the development of acute kidney injury. Design: Cohort study of critically ill children. Setting: University-affiliated PICU. Patients: Eligible patients were admitted to the ICU between January 2006 and June 2009. We excluded those admitted with known primary renal failure, chronic renal failur...
