Candidemia Diagnosis With T2 Nuclear Magnetic Resonance in a PICU: A New Approach

CONCLUSIONS: T2Candida might be useful for the diagnosis of candidemia in PICUs. The prevalence of candidemia might be underestimated in this pediatric population. The use of this diagnostic tool in these units may help clinicians to start adequate and timely antifungal treatments.

We report a case of invasive candidiasis causing a mycotic pseudoaneurysm involving the ascending aorta and pulmonary artery. The patient presented two years after heart ‐kidney transplant with acute limb ischemia resulting from embolization of fungal vegetations. Abstract Five C. krusei isolates (susceptible and resistant) recovered from the urine of a kidney transplant patient treated with VRC 200mg/2x day for 20 days were studied. Eight unrelated clinical isolates of C. krusei were exposed in vitro to VRC 0.001 μg/ml, during 30 days. Development of VRC transient resistance occurred in vivo and induction of permanent resistance occurred in vitro Mostly ABC1 and ERG11 genes were overexpressed and a homozygous T418C mutation in ERG11 gene was found. A 54-year-old white female patient presented with bilateral angular cheilitis and several painful ulcerated oral lesions, 11 years after kidney transplantation. The lesions affected the buccal mucosa, the soft palate, and dorsum of the tongue for 8 months. Her current immunosuppressive regimen includes mycophenolate mofetil, tacrolimus, and prednisone. Periodic acid-Schiff (PAS) exfoliative cytology was positive for Candida species. An incisional biopsy was performed on the tongue lesion and the histopathologic examination revealed intense mixed inflammatory infiltrate in the connective tissue.

We present an 18 year-old adolescent with juvenile onset SIOD who was diagnosed after renal transplantation and developed BK virus associated nephropathy (BKVAN) and severe pneumonia during follow-up. The patient developed nephrotic syndrome, unresponsive to immunosuppressives, at the age of 8 years. In conclusion, circulating MDSCs are measurable, functional and have a G-MDSC phenotype in lung transplant patients.
