Optimizing Care in Kidney Transplantation
This article provides fundamental information to health care providers on kidney transplant.  It reviews some of the preoperative testing, postoperative complications, and treatments. It is to help those who help assist with postoperative care of kidney transplants ensure care is optimal. This article will help educate and inform caregivers of guidelines and suggested plans of care to foll ow for best outcomes for transplant recipients. Preliminary evaluation of tear production in dogs hospitalized in an intensive care unit
ConclusionsThis study demonstrates a decrease in tear production in canine ICU patients. While further study is warranted to determine how different diseases impact tear production, these finding support the implementation of frequent ocular lubrication in all ICU patients.

Considerations for a Primary Care Physician Assistant in Treating Kidney Transplant Recipients.
Abstract The escalating amount of kidney transplant recipients (KTRs) represents a significant dilemma for primary care providers. As the number of physician assistants (PAs) has been steadily increasing in primary care in the United States, the utilization of these healthcare professionals presents a solution for the care of post-kidney transplant recipients. A physician assistant (PA) is a state licensed healthcare professional who practices medicine under physician supervision and can alleviate some of the increasing demands for primary patient care. Here we provide an outline of the crucial components and cons...

Continuous renal replacement therapy and its impact on hyperammonaemia in acute liver failure.
CONCLUSION: In Australian and New Zealand patients with ALF, CRRT is typically started early, before Stage 3 AKI or severe acidaemia, and in the presence hyperammonaemia. In these more severely ill patients, CRRT use was associated with prevention of extreme hyperammonaemia, which in turn, was associated with increased transplant-free survival. Timing of renal-replacement therapy in intensive care unit-related acute kidney injury
Purpose of review The optimal timing of renal-replacement therapy (RRT) initiation for the management of acute kidney injury (AKI) in the intensive care unit (ICU) is frequently controversial. An earlier-strategy has biological rationale, even in the absence of urgent indications; however, a delayed-strategy may prevent selected patients from receiving RRT and avoid complications related to RRT. Recent findings Previous studies assessing the optimal timing of RRT initiation found conflicting results, contributing to variation in clinical practice. The recent multinational trial, standard vs. accelerated initiati...

Nonsurgical Care of Intestinal and Multivisceral Transplant Recipients: A Review for the Intensivist
This article is intended for intensivists who provide care to critically ill recipients of intestinal and multivisceral transplants. As perioperative care of intestinal/multivisceral transplant recipients has been described elsewhere, this review focuses on common nonsurgical complications with which one should be familiar in order to provide optimal care. The article is both a review of the current literature on multivisceral and isolated intestinal transplantation as well as a reflection of our own experience at the University of Miami.

Jellybean 76 with Rinaldo Bellomo
LITFL • Life in the Fast Lane Medical Blog LITFL • Life in the Fast Lane Medical Blog - Emergency medicine and critical care medical education blog What is the future of Artificial Intelligence and critical care medicine? I ask Rinaldo Bellomo who, for the past 30 years, has been at the pointy end of critical care research including high technology and predictive algorithms. I managed to get Rinaldo Bellomo to talk to me at SMACC in Berlin. It was a bit of an intellectual mismatch. If you work in intensive care then you know who this man is. If you don’t work in intensive care you may somehow be unaware of the ...

Pediatric solid organ transplant recipients: Transition to home and chronic illness care
Abstract Pediatric SOT recipients are medically fragile and present with complex care issues requiring high‐level management at home. Parents of hospitalized children have reported inadequate preparation for discharge, resulting in problems transitioning from hospital to home and independently self‐managing their child's complex care needs. The aim of this study was to investigate factors associated with the transition from hospital to home and chronic illness care for parents of heart, kidney, liver, lung, or multivisceral recipients. Fifty‐one parents from five pediatric transplant centers completed questionnaires ...

Overview of the Medical Management of the Critically Ill Patient
Clin J Am Soc Nephrol. 2022 Nov 18:CJN.07130622. doi: 10.2215/CJN.07130622. Online ahead of print.ABSTRACTThe medical management of the critically ill patient focuses predominantly on treatment of the underlying condition (e g, sepsis or respiratory failure). However, in the past decade, the importance of initiating early prophylactic treatment for complications arising from care in the intensive care unit setting has become increasingly apparent. As survival from critical illness has improved, there is an increased prevalence of postintensive care syndrome-defined as a decline in physical, cognitive, or psychologic functi...
