This study showed that in more than 4 of 5 obese children, the subcutaneous fat layer was too thick for an IM injection one quarter of the way down the thigh. In nearly 1 of 5 obese children, the subcutaneous fat layer was too thick three quarters of the way down the thigh. In those patients, the study suggested injecting the calf. I suppose the manufacturer could be forced to make autoinjectors with longer needles, but then non-obese patients would theoretically risk getting a bone marrow injection of epinephrine. Clinical Reasoning: An 85-year-old man with paresthesias and an unsteady gait
An 85-year-old man developed tingling in his feet, followed 1 week later by a similar sensation in both hands. He reported difficulty buttoning his shirt and unsteadiness when walking. There was no ascent of his symptoms proximal to the wrists and ankles. He denied pain, orthostasis, and bowel or bladder symptoms. He had had no prior similar symptoms, preceding illnesses, or recent changes in his health or medications. He had received the influenza vaccine 1 week prior to symptom onset.

A 7-year-old boy with a history of recurrent acute lymphoblastic leukemia (ALL), in remission, presented to primary care clinic after 2 days of progressive right hip pain with weight-bearing activities. He was otherwise asymptomatic at the time of presentation. Blood cultures revealed Gram-negative diplococci, which prompted an MRI that was significant for a hip joint effusion and femoral head bone marrow edema. Impact of Voluntary Accreditation on Short-Stay Rehabilitative Measures in U.S. Nursing Homes.
CONCLUSIONS: Approaches beyond traditional regulation and governmental inspections are necessary to improve the quality of care in nursing homes. CLINICAL RELEVANCE: During a patient's rehabilitation stay, minimizing iatrogenic complications is paramount. Pain-free microneedle influenza vaccine is effective, long-lasting
(American Society for Microbiology) Scientists have developed an influenza vaccine delivered via microneedle patch that provided 100 percent protection against a lethal influenza virus in mice more than one year after vaccination.
