Addressing health care inequities in Canadian critical care through inclusive science: a pilot tool for standardized data collection
CONCLUSION: We developed a standardized, practical, and convenient demographic data collection tool for critical care research studies. Questions and response options can be adapted by researchers for inclusion in individual study questionnaires or case report forms.PMID:37165123 | DOI:10.1007/s12630-023-02450-1

End-of-Life Care in Taiwan: Single-Center Retrospective Study of Modes of Death*
This study describes change in modes of death and end-of-life care in a single center, 2011–2017. SETTING: Multidisciplinary PICU in a tertiary referral Children’s Hospital in Taiwan. PATIENTS: There were 316 deaths in PICU patients. INTERVENTIONS: Palliative care consultation in the PICU service occurred after the 2013 “Hospice Palliative Care Act” revision. MEASUREMENTS AND MAIN RESULTS: In the whole cohort, 22 of 316 patients (7%) were determined as “death by neurologic criteria”. There were 94 of 316 patients (30%) who had an event needing cardiopulmonary resuscitation within 24 ...

Financial outcomes after pediatric critical illness among commercially insured families
Critical illness results in subjective financial distress for families, but little is known about objective caregiver finances after a child ’s pediatric intensive care unit (PICU) hospitalization. Using statew...

Contemporary Postnatal Incidence of Acquiring Acute Myocarditis by Age 15 Years and the Outcomes From a Nationwide Birth Cohort
Conclusions: This birth cohort study determined the cumulative incidence of acute myocarditis for neonates by 15 years old to be one in 2,105. In an era of extracorporeal membrane oxygenation, the need of extracorporeal membrane oxygenation may reflect the severity of acute myocarditis and predict its outcome.

Critical care medicine in Taiwan from 1997 to 2013 under National Health Insurance.
Conclusions: ICU admissions significantly increased from 1997 to 2013, especially for elderly patients, in contrast, the mortality rate of ICU patients significantly declined with time. In addition, the ICU LOS did not change during the study period. PMID: 30233870 [PubMed]

Factors affecting early treatment goals of sepsis patients presenting to the emergency department
Conclusion: The sepsis bundle goals were not achieved because of various factors such as under triaging, delay in diagnosis, logistical delay, and financial constraints. Further studies on whether sensitization of medical fraternity about sepsis, implementation of insurance policies for patient care or better point of care diagnostics would aid in achieving the bundles may be evaluated further.

Investigations of the Pediatric Hospice Care in Taiwan: 2005 to 2010.
CONCLUSIONS: Pediatric hospice care revealed characteristics different from adults. Underlying late referrals were prevalent. There is space to promote the utilization of hospices for terminally ill pediatric patients. PMID: 23739240 [PubMed - as supplied by publisher]

Medical Home and Pediatric Primary Care Utilization Among Children With Special Health Care Needs
This study examined the relationship between the presence of a medical home and pediatric primary care office visits by children with special health care needs (CSHCN) using the data from 2005-2006 National Survey of Children with Special Healthcare Needs.Method: Survey logistic regression was used to analyze the relationship. Results: When CSHCN age, gender, ethnicity/race, functional status, insurance status, household education, residence, and income were included in the model, CSHCN with a medical home were 1.6 times more likely to have six or more annual pediatric primary care office visits than were children without ...

Case Study: Daily review of telemonitoring data by a critical care nurse improves outcomes in a patient with Class IV, Stage D heart failure with reduced ejection fraction receiving home intravenous dobutamine for palliation.
ConclusionsDaily critical care nurse telemonitoring resulted in nearly daily interventions with gradual stabilization of weight and vital signs, no decline in respiratory status, and no hospitalizations throughout 69 days of service. Of particular significance is the fact that telemonitoring improved the patient's compliance to his treatment plan, which resulted in an evaluation for a left ventricular assist device placement.

Risk factors for avoidable transfer to a pediatric trauma center among patients 2 years and older
CONCLUSION Among injured children 2 years and older, those with minor head injuries were at greatest risk for avoidable transfer. Many were transferred because of a perceived need for evaluation by a pediatric neurosurgeon. Future projects seeking to reduce avoidable transfers should focus on children with isolated skull fractures and concussions, in whom there is no suspicion of nonaccidental trauma. LEVEL OF EVIDENCE Therapeutic/care management, level IV.

Age-Specific Distribution of Diagnosis and Outcomes of Children Admitted to ICUs: A Population-Based Cohort Study*
Objectives: Although several studies have reported outcome data on critically ill children, detailed reports by age are not available. We aimed to evaluate the age-specific estimates of trends in causes of diagnosis, procedures, and outcomes of pediatric admissions to ICUs in a national representative sample. Design: A population-based retrospective cohort study. Setting: Three hundred forty-four hospitals in South Korea. Patients: All pediatric admissions to ICUs in Korea from August 1, 2009, to September 30, 2014, were covered by the Korean National Health Insurance Corporation, with virtually complete coverag...

Telemedicine for Interfacility Nurse Handoffs*
Conclusions: Telemedicine is feasible for nurse-to-nurse handoffs of critically ill patients between pretransfer and receiving facilities and may be associated with increased perceived and objective nurse preparedness upon patient arrival. Additional research is needed to demonstrate that telemedicine during nurse handoffs improves communication, decreases preventable adverse events, and impacts family and provider satisfaction.
