Liver Transplant in Hemoglobin SC Disease and Autoimmune Hepatitis: A Case Report
We report a case of a 50-year-old man with sickle cell disease genotype SC (HbSC) and cirrhosis secondary to autoimmune hepatitis who underwent liver transplant. His postoperative course included upper extremity deep vein thrombosis, pulmonary embolus, stroke via a patent foramen ovale after a line removal, and posterior reversible encephalopathy syndrome. Fortunately, he is alive with a functioning graft at 10 months after liver transplant. This case highlights the feasibility of liver transplant in sickle cell disease given the support of meticulous multidisciplinary care and the unique aspects of autoimmune hepatitis an...

Discovery of a Rare Variant of the Arc of B ühler During Liver Procurement
We report a rare variant of the hepatic arterial supply observed during liver procurement from a 71-year-old female donor for whom an ischemic stroke caused brain death. Preoperative computed tomography showed a partially obliterated celiac trunk and an atypical arterial branch that coursed in a retropancreatic plane away from its origin at the superior mesenteric artery to anastomose with the common hepatic artery at the origin of the proper hepatic artery. The gastroduodenal artery and the dorsal pancreatic artery are the 2 conventional anastomotic arcades between the celiac trunk and the superior mesenteric artery. Howe...

Major Thromboembolic Complications in Liver Transplantation: The Role of Rotational Thromboelastometry and Cryoprecipitate Transfusion
Background. Although hemorrhage is a major concern during liver transplantation (LT), the risk for thromboembolism is well recognized. Implementation of rotational thromboelastometry (ROTEM) has been associated with the increased use of cryoprecipitate; however, the role of ROTEM-guided transfusion strategy and cryoprecipitate administration in the development of major thromboembolic complications (MTCs) has never been documented. Methods. We conducted a study on patients undergoing LT before and after the implementation of ROTEM. We defined MTC as intracardiac thrombus, pulmonary embolism, hepatic artery thromb...

Cardiac Transplantation for Cancer Involving the Heart
Cardiac Cancer (CC) that arises from or involves the heart can present as heart failure, chest pain, stroke or another thromboembolic event. Although patients with CC have limited treatment options, in other organ cancers such as hepatocellular carcinoma, liver transplant improves 5-year overall survival in carefully selected patients1. For CC, however, the benefit of replacing the cancerous heart is uncertain because one-year survival is generally estimated around 50% after this rare operation.

Intraoperative Hemodynamic Parameters and Acute Kidney Injury After Living Donor Liver Transplantation
Conclusions. The elevated baseline CVP, elevated baseline RVEDV after anesthesia induction, and decreased SvO2 during anhepatic phase were associated with postoperative AKI. Prospective trials are required to evaluate whether the optimization of these variables may decrease the risk of AKI after LDLT.

VCA Deceased Donors in the United States
Conclusions. donors are a demographically and clinically diverse group. Understanding this diversity and future trends in VCA donor characteristics is critical in supporting this life-changing field of transplantation.

Heat Stroke as a Cause of Liver Failure and Evaluation of Liver Transplant.
Abstract Heat stroke is a multiple organ dysfunction syndrome of poorly understood pathogenesis. Exertional heat stroke with acute liver failure is a rarely reported condition. Liver transplant has been recommended as treatment in cases of severe liver dysfunction; however, there are only 5 described cases of long-term survival after this procedure in patients with heat stroke. Here, we present 2 cases of young athletes who developed heat stroke. Both patients developed acute liver failure and were listed for liver transplant. Liver function tests of one patient improved, and he was discharged on postoperative day...

Cardiovascular Disease Outcomes Related to Early Stage Renal Impairment After Liver Transplantation
Background In the general population, even mild renal disease is associated with increased cardiovascular (CV) complications. Whether this is true in liver transplant recipients (LTR) is unknown. Methods This was a retrospective cohort study of 671 LTR (2002-2012) from a large urban tertiary care center and 37 322 LTR using Vizient hospitalization data linked to the United Network for Organ Sharing. The 4-variable Modification of Diet in Renal Disease equation estimated glomerular filtration rate (eGFR). Outcomes were 1-year CV complications (death/hospitalization from myocardial infarction, heart failure, atrial fibr...

The correlation between CVP and SVV and intraoperative minimal blood loss in living donor hepatectomy
Blood loss in liver surgery is found to be correlated with central venous pressure (CVP). The aim of the current retrospective study is to find out the cut of value of CVP and stroke volume variation (SVV), which may increase the risk of having intraoperative blood loss more than 100 ml during living liver donor hepatectomy.

Hospitalizations for cardiovascular disease after liver transplantation in the United States
Conclusions: CVD after liver transplant is evolving and responsible for growing rates of inpatient care. Transplant hospitals are associated with poor outcomes, even after risk adjustment for patient and hospital characteristics, which may be attributable to selective referral of certain patient phenotypes, but could also be related to differences in quality of care. Further study is warranted. This article is protected by copyright. All rights reserved.
