Cognition and Cerebral Infarction in Older Adults After Surgical Aortic Valve Replacement
ConclusionsIn high-risk, aged participants undergoing surgical AVR for aortic stenosis, post-operative cognitive dysfunction was surprisingly limited and was resolved by 1-year in most. Post-operative cognitive dysfunction at 4-6 weeks was associated with more and larger acute cerebral infarcts.

Antiplatelet strategy after transcatheter aortic valve replacement: an updated meta-analysis.
CONCLUSIONS: The updated meta-analysis presents the evidence that post-TAVR DAPT increases bleeding events, with no benefit in survival and ischemic events, in comparison with SAPT. Nevertheless, it is currently difficult to evaluate by a meta-analysis the effectiveness of DAPT versus SAPT to prevent the valve thrombosis resulting in leaflet dysfunction , due to a limited number of existing publication. Mitral Valve Replacement Using Subvalvular Apparatus: A Systematic Review and Meta-Analysis.
CONCLUSION: MVR with the preservation of subvalvular apparatus improves clinical outcomes, such as long-term mortality, hospital length of stay, pneumonia, and bleeding. There is no significant difference in the risk of stroke, renal failure, or ICU length of stay. Surgical vs transfemoral aortic valve replacement in low ‐risk patients: An updated meta‐analysis of trial and registry data
ConclusionsEarly mortality is higher in SAVR while rates of PVL and PPMI are substantially higher in TAVR. There is no significant advantage with TAVR for overall survival, cardiovascular death, stroke, MI, and re ‐intervention rates.

CONCLUSIONS: Long-term survival, overall reoperation, or stroke incidence was comparable among the two groups, while patients with a mechanical valve showed a greater likelihood of major bleeding events. Impact of left ventricle outflow tract calcification on the outcomes of transcatheter aortic valve implantation: A study ‐level meta‐analysis
ConclusionThe presence of significant LVOT calcification increases the risk of periprocedural and 1-year mortality, stroke, myocardial infarction, PVL and aortic annulus/root rupture after TAVI. Self-expandable valves may be a preferrable option in this scenario. Structural heart surgeons and interventional cardiologists should consider this factor when referring patients for TAVI and technical aspects (such as the type of transcatheter heart valve to be deployed or the use of pre-/post-dilatation) should be factored in.
