Low CD8 T‐cells in neonates and infants prior to surgery and healthcare‐associated infections: a prospective observational study.
Cytotoxic T‐lymphocytes (CD8 T‐cells) are a powerful component of the adaptive immune system. They are involved in the eradication of intracellular infections, tumours, and contribute to rejection of organ transplants; and autoimmunity [1]. Several studies in humans and animals have demonstrated that altered host defence mechanisms after major surgery or trauma are an important risk in the development of postsurgical healthcare‐associated infections [2‐4]. These and other studies have demonstrated decreased counts and/or response of T‐lymphocytes to mitogenic activation in patients following general surgery, blun...

Effects of pneumoperitoneum induced at various pressures on cardiorespiratory function and working space during laparoscopy in cats.
Conclusions and Clinical Relevance-Values of cardiopulmonary variables were largely unchanged by induction of pneumoperitoneum in healthy cats up to an IAP of 8 mm Hg, and no clinically important increases in working space were evident at an IAP of 15 versus 8 mm Hg. These findings provide little justification for use of IAPs > 8 mm Hg in healthy cats undergoing laparoscopic procedures; however, whether the situation is similar in diseased or elderly cats remains to be determined. Safety of Outpatient Laparoscopic Cholecystectomy in the Elderly: Analysis of 15,248 Patients Using the NSQIP Database
Background: Studies have shown that laparoscopic cholecystectomy (LC) in an ambulatory setting is a safe alternative to the traditional overnight hospital stay. However, there are limited data on the morbidity and mortality of outpatient LC in elderly patients. We evaluated the safety of ambulatory LC in the elderly and identified risk factors that predict inpatient admission.Study Design: A retrospective analysis was performed using the American College of Surgeon's NSQIP database between 2007 and 2010. The database was searched for patients older than 65 years of age who underwent elective LC at all participating hospita...

Gastroesophageal Reflux in Chronic Cough and Cough Syncope and the Effect of Antireflux Treatment: Case Report and Literature Review.
CONCLUSION: For chronic cough and cough syncope of unknown cause, the GER assessment could be valuable. In treating well-selected GER-related chronic cough and cough syncope, PPI, SRF, and LF can be considered. Moreover, satisfactory restoration of physical and social functions could be achieved after effective antireflux therapy. PMID: 24842868 [PubMed - as supplied by publisher]

Introduction of oesophageal Doppler-guided fluid management in a laparoscopic colorectal surgery enhanced recovery programme: an audit of effect on patient outcome.
Abstract Morbidity after colorectal surgery can be reduced with intraoperative oesophageal Doppler monitor (ODM) guided fluid therapy. We audited the effect of introducing ODM-guided fluid therapy in enhanced recovery laparoscopic colorectal surgery. ODM group (n = 40) outcomes (toleration of diet, Post Operative Morbidity Survery (POMS) score, complications) were compared to matched patients (n = 40) who had the same surgery using a conventional approach to fluid management. Mean (SD) time to tolerate diet was shorter in the ODM group (2.3 (1.6) days vs 3.8 (2.4) days, p = 0.003). The ODM group had a lower mean (...

Factors Associated with Conversion from Laparoscopic to Open Colectomy using the National Surgical Quality Improvement Program (NSQIP) Database
ConclusionsMultiple significant factors for conversion from laparoscopic to open colectomy were identified. A novel finding was the increased risk of conversion for underweight patients. As laparoscopic colectomy is become increasingly utilized, factors predictive of conversion to open procedures should be sought via large, national cohorts. This article is protected by copyright. Intraoperative circulatory management using the FloTracTM system in laparoscopic liver resection
ConclusionThe average SVV value during laparoscopic liver transection (mean, 17.0%) exceeded the conventional cut‐off value, but in this study, no perioperative complications developed, which enabled safe management. We might be able to manage appropriate fluid control using FloTrac system in patients with laparoscopic liver resection. Therefore, it is necessary to set the target SVV and conduct prospective trials to verify the safety margin for intraoperative management in the future.

Board #153 - Research Abstract The Creation of a Simulator Value Index Tool by Connected Consensus (Submission #8590).
Conclusion: The product from this work (SVI Worksheet tool) can be applied aid the simulator purchase process across countries, institutions, and stakeholder. We are hopeful that simulator purchase processes might include the SVI Worksheet with the intention to maintain alignment with educational goals, while still accounting for other factors considered at the institutional level. At the same time, common measures might facilitate future communication across academic medical institutions and simulator industry. More evaluation is required following broad-scaled implementation. Pain Management Strategies for Urogynecologic Surgery: A Review
ABSTRACT: Surgery-related pain is common and is difficult to manage during the postoperative period. In some studies, more than 30% of patients who underwent either vaginal surgery or abdominal surgery reported inadequate pain control at postoperative days 3 and 7. Poor pain control during the acute postoperative period causes heightened sympathetic discharge (increasing the risk for myocardial infarction and stroke) and decreased ambulation (increasing the risk for thromboembolism). Poor acute postoperative pain control can also result in poor inspiratory effort, resulting in an increased risk for postoperative pneumonia,...

Roux-en-Y gastric bypass alleviates hypertension and is associated with an increase in mid-regional pro-atrial natriuretic peptide in morbid obese patients
Conclusions: LRYGB resulted in a significant 24BP reduction and a substantial increase in MRproANP plasma concentrations in hypertensive, obese patients 6 weeks after surgery, suggesting a causal link between obesity–hypertension and altered release/degradation of cardiac natriuretic peptides.
