Ventricular energetics early after surgery for chronic mitral regurgitation: repair versus replacement.
CONCLUSION: Compensation of LV geometry after correction of chronic MR preserved ventricular contractility. Furthermore, the results of MV repair were not superior to those of MV replacement with partial chordal preservation in the early postoperative period. This suggested that partial chordal-sparing MV replacement is an effective method for the treatment of chronic MR in selected patients. PMID: 24597401 [PubMed - indexed for MEDLINE]

Postoperative warfarin following mitral valve repair or bioprosthetic valve replacement.
CONCLUSION: The use of postoperative warfarin following MVR does not reduce the incidence of stroke at early follow up. However, there remains a trend for improved long-term outcomes in those patients receiving postoperative warfarin therapy. PMID: 24383387 [PubMed - indexed for MEDLINE]

Propensity-Matched Comparisons of Clinical Outcomes after Transapical or Transfemoral TAVR: A PARTNER-I Trial Substudy.
CONCLUSIONS: -Likelihood of adverse periprocedural events and prolonged recovery is greater after TA-TAVR than TF-TAVR in vasculopathic patients after accounting for differences in cardiovascular risk factors, although stroke risk is equivalent and aortic regurgitation less. As smaller delivery systems permit TF-TAVR in many of these patients, we recommend a TF-first access strategy for TAVR when anatomically feasible. Clinical Trial Registration-www.clinicaltrials.gov. Abstract 234: Comparison of Hospital Length of Stay and Costs between Non-valvular Atrial Fibrillation Patients Treated with Either Apixaban or Warfarin Session Title: Poster Session II
Conclusions: NVAF patients treated with apixaban had significantly shorter hospital LOS and lower index hospitalization costs compared to those treated with warfarin. Costs remained significantly lower for apixaban patients across all CHADS2 scores.

Conclusions: Practice and outcomes in TAVI vary widely across Quebec hospitals and internationally. Despite publication of recommended endpoints (VARC and BARC), there is a lack of standardized reported outcomes and patient populations. While quality benchmarks for patient selection and outcomes remain unclear, continued monitoring with timely feedback to TAVI teams and decision-makers is essential for this new and costly intervention that is predominantly being performed in the very elderly.

Complications and Mortality in Patients Undergoing Transcatheter Aortic Valve Replacement With Edwards SAPIEN & SAPIEN XT Valves: A Meta‐Analysis of World‐Wide Studies and Registries Comparing the Transapical and Transfemoral Accesses
ConclusionThis meta‐analysis demonstrates a decreased 30‐day and 1‐year mortality in TF TAVR as compared to TA TAVR. Post‐procedure acute kidney injury and the need for renal replacement therapy are also significantly lower in the TF group. These differences hold true even after utilizing the standardized Valve Academic Research Consortium criteria.

Procedural Results and Clinical Outcomes of Transcatheter Aortic Valve Implantation in Switzerland: An Observational Cohort Study of Sapien 3 Versus Sapien XT Transcatheter Heart Valves Structural Heart Disease
Conclusions— The use of the new generation S3 balloon-expandable THV reduced the risk of more than mild paravalvular regurgitation and vascular complications but was associated with an increased permanent pacemaker rate compared with the XT. Transcatheter aortic valve implantation using the newest generation balloon-expandable THV is associated with a low risk of stroke and favorable clinical outcomes.
