Primary stroke prevention in children with sickle cell anemia living in africa: the false choice between patient-oriented research and humanitarian service-part ii
Trans Am Clin Climatol Assoc. 2022;132:236-248.ABSTRACTIn the United States, primary stroke prevention in children with sickle cell anemia (SCA) is the standard of care and includes annual transcranial Doppler ultrasound to detect elevated velocities. For children with velocities ≥200 cm/sec, initial monthly blood transfusion therapy for at least a year, followed by the option of hydroxyurea therapy, results in a significant decline in the stroke rate. In Africa, no prior strategy exists for primary stroke prevention because regular blood transfusion therapy is not feasible. We completed a randomized controlled trial for...

Chronic blood transfusion for primary and secondary stroke prevention in Nigerian children with sickle cell disease: A 5‐year appraisal
ConclusionHigh economic costs, unavailability of blood, need to regularly seek for blood donors, cultural beliefs, and high frequency of transfusion reactions are major challenges to a successful CBT program in Nigeria. There is a need for government subsidy on blood transfusions and improved efforts towards provision of safe and affordable blood. Pediatr Blood Cancer. © 2013 Wiley Periodicals, Inc.

O-001 groin complications in endovascular mechanical thrombectomy for acute ischemic stroke: a 10-year single center experience
ConclusionsIn our series, the low groin complication rate (0.4%-0.8%) in thrombectomy suggests that concerns for vascular complications should not preclude the use of BGCs in the endovascular treatment of ischemic stroke.DisclosuresV. The Way Forward: Challenges and Opportunities in Pediatric Stroke
The past two decades have seen a remarkable increase in publications on pediatric stroke, reflecting increased awareness of its impact on childhood health. The results of the Stroke Prevention Trial in Sickle Cell Anemia (STOP), published in 1998, which showed that chronic blood transfusion decreased the risk of stroke in high risk children with sickle cell anemia by 92%, revolutionized the care of children with sickle cell anemia 1 and highlighted the importance of primary stroke prevention. Surgical revascularization is used increasingly to decrease the risk of ischemic and hemorrhagic stroke in children with moyamoya ce...

Primary stroke prevention in children with sickle cell anemia living in africa: the false choice between patient-oriented research and humanitarian service.
PRIMARY STROKE PREVENTION IN CHILDREN WITH SICKLE CELL ANEMIA LIVING IN AFRICA: THE FALSE CHOICE BETWEEN PATIENT-ORIENTED RESEARCH AND HUMANITARIAN SERVICE. Trans Am Clin Climatol Assoc. 2016;127:17-33 Authors: Debaun MR, Galadanci NA, Kassim AA, Jordan LC, Phillips S, Aliyu MH Abstract In the United States, primary stroke prevention in children with sickle cell anemia (SCA) is now the standard of care and includes annual transcranial Doppler ultrasound evaluation to detect elevated intracranial velocities; and for those at risk, monthly blood transfusion therapy for at least a year followed by the opt...

Ischemic Stroke Related to Severe Iron-Deficiency Anemia in Adults May Benefit from Blood Transfusion (P01.232)
CONCLUSIONS: Prevalence of IDA is 1-20%; it is 3.9% in our AIS population and is often related to untreated uterine bleeding. As described in the pediatric population, severe IDA may be an underlying mechanism of ischemic stroke in adults, therefore, efforts must be undertaken to identify and treat the cause of the anemia. While patients with AIS and severe IDA may receive TPA, blood transfusion may be considered as an adjuvant therapy.Supported by: SPOTRIAS P50 NS044227 & NIH Training Grant 5T 32NS0077412-12.Disclosure: Dr. Choi has nothing to disclose. Pain and other non‐neurological adverse events in children with sickle cell anemia and previous stroke who received hydroxyurea and phlebotomy or chronic transfusions and chelation: Results from the SWiTCH clinical trial
Conclusions: In children with SCA and prior stroke, monthly transfusions and daily iron chelation provided superior protection against acute vaso‐occlusive pain SAEs when compared to hydroxyurea and monthly phlebotomy.

Intravenous Tissue Plasminogen Activator for an Ischemic Stroke with Occult Double Primary Cancer
Discussion: Although systemic thrombolysis with tPA for ischemic stroke in patients with advanced-stage cancer may be performed relatively safely, optimal post-thrombolysis management is important to prevent the complications.Case Rep Neurol 2014;6:238-242

Effective evaluation of arterial pulse waveform analysis by two-dimensional stroke volume variation –stroke volume index plots
In this study, we analyzed how we visually interpret hemodynamic data, including stroke volume variation (SVV) and stroke volume (SV) derived from APWA. We performed arithmetic estimation of the SVV –SV relationship and applied measured values to this estimation. We then collected measured values in six anesthesia cases, including three liver transplantations and three other types of surgeries, to apply them to this SVV–SVI (stroke volume variation index) plot. Arithmetic analysis showed th at the relationship between SVV and SV can be drawn as hyperbolic curves. Plotting SVV-SV values in the semi-logarithmic scale sho...

Incidence of perioperative stroke in clampless aortic anastomosis during off-pump coronary artery bypass grafting
This study aimed to assess if clampless off-pump coronary artery bypass grafting (OPCAB) decreases the incidence of perioperative stroke (POS) rate and in-hospital mortality. The secondary aim was to evaluate 12-year rates of overall mortality. Between January 2003 to December 2015, data of 645 consecutive patients undergoing isolated CABG were retrospectively collected. 363 underwent aortic no-touch OPCAB (No-touch group) and 282 underwent OPCAB with the Heartstring device (HS group). In-hospital mortality and perioperative stroke rate as primary endpoint, as well as long-term follow-up outcome were analysed. In-hospital ...

Stroke and presence of patent foramen ovale in sickle cell disease
AbstractSickle cell disease (SCD) is an inherited monogenic hemoglobinopathy characterized by formation of sickle erythrocytes under conditions of deoxygenation. Sickle erythrocytes can lead to thrombus formation and vaso-occlusive episodes that may result in hemolytic anemia, pain crisis and multiple organ damage. Moreover, SCD is characterized by endothelial damage, increased inflammatory response, platelet activation and aggravation, and activation of both the intrinsic and the extrinsic coagulation pathways. Cerebrovascular events constitute an important clinical complication of SCD. Children with SCD have a 300-fold h...

Dilemma in approach to stroke in sickle cell disease patient: A case report
Conclusion: Recommendations need to be put in place regarding screening for PFO in patients with SCD, in addition to highlighting issues of whether screening needs to be done in patients who have not developed stroke, and if PFO were to be found, what would be the best management approach and how will prognosis be affected.

The interaction effect of transfusion history and previous stroke history on the risk of venous thromboembolism in stroke patients: a prospective cohort study
Blood transfusion and previous stroke history are two independent risk factors of venous thromboembolism (VTE) in stroke patients. Whether the potential interaction of transfusion history and previous stroke h...
