Abstract 237: Anticoagulant Use For the Prevention of Stroke in Patients with Atrial Fibrillation: Findings From a Multi-Payer Analysis Poster Session II
Conclusions: Many AF patients in selected commercial, Medicare-eligible, and Medicaid populations, including those at high risk of stroke, do not receive appropriate thromboprophylaxis, as recommended by treatment guidelines. Increased use of the analyzer and similar software may support enhanced education efforts aimed at improving adherence to guidelines and quality of care.

Exploring Overlaps Between the Genomic and Environmental Determinants of LVH and Stroke
Conclusions The prevalence of LVH was high among stroke patients especially the younger ones, suggesting a genetic component to LVH. Hypertension was a major modifiable risk factor for stroke as well as LVH. It is envisaged that the SIREN project will elucidate polygenic overlap (if present) between LVH and stroke among Africans, thereby defining the role of LVH as a putative intermediate cardiovascular phenotype and therapeutic target to inform interventions to reduce stroke risk in populations of African ancestry.

Conclusions: Rehabilitation needs were documented for patients with minor stroke and almost one third were discharged to receive inpatient rehabilitation in an IRF or SNF. Taking pre-stroke disability into consideration for transition planning from acute care could facilitate earlier communication and planning with the next point of care.

Conclusions: Many stroke patients do not activate 911 or arrive to a hospital in a timely manner. Collaborative attempts by state partners to provide education to the public has not made much difference in arrival mode and arrival from time last known well. Structured outreach activities utilizing initiatives such as the American Heart Association’s signs and symptoms materials can continue to be utilized, but a more comprehensive plan among state stakeholders, EMS, hospitals and the AHA will need to be developed to improve patient awareness and subsequently patient arrival times, resulting in better outcomes.

The Risk Stratification and Stroke Prevention Therapy Care Gap in Canadian Atrial Fibrillation Patients
Conclusions In a large Canadian AF population, primary care physicians did not provide a stroke or bleeding risk in a substantial proportion of their AF patients. When estimates were provided, they were on the basis of a predictive stroke and bleeding risk index in less than half of the patients. Furthermore, there was under- and overestimation of stroke and bleeding risk in a substantial proportion of patients. As many as 1 in 3 patients receiving warfarin have their TTR < 60%. These findings suggest an opportunity to enhance knowledge translation to primary care physicians.
