Severity Assessment and the Immediate and Long-Term Prognosis in Community-Acquired Pneumonia
Semin Respir Crit Care Med 2016; 37: 886-896 DOI: 10.1055/s-0036-1592127Severity assessment is a crucial step in the initial management of patients with community-acquired pneumonia (CAP). While approximately half of patients are at low risk of death and can be safely treated as outpatients, around 20% are at increased risk. While CURB-65 (confusion, respiratory rate, blood pressure, urea) and pneumonia severity index (PSI) scores are equally useful as an adjunct to clinical judgment to identify patients at low risk, the so-called minor American Thoracic Society/Infectious Diseases Society of America criteria are predictiv...

Palliative Care Exposure in Internal Medicine Residency Education.
In this study, we surveyed 338 Accreditation Council for Graduate Medical Education-accredited internal medicine program directors. We queried how palliative care was integrated into their training programs. The vast majority of respondents felt that palliative care training was "very important" (87.5%) and 75.9% of respondents offered some kind of palliative care rotation, often with a multidisciplinary approach. Moving forward, we are hopeful that the data provided from our survey will act as a launching point for more formal investigations into palliative care education for internal medicine residents. Concurrently, pol...

Health Care Needs Its Rosa Parks Moment
BY SHANNON BROWNLEE On Wednesday, October 25, 2017 I was at the inaugural Society for Participatory Medicine conference. It was a fantastic day and the ending keynote was the superb Shannon Brownlee. It was great to catch up with her and I’m grateful that she agreed to let THCB publish her speech. Settle back with a cup of coffee (or as it’s Thanksgiving, perhaps something stronger), and enjoy–Matthew Holt George Burns once said, the secret to a good sermon is to have a good beginning and a good ending—and to have the two as close together as possible. I think the same is true of final keynotes after a...

Anesthesiology or Critical Care?
Question guys, first off excuse my ignorance because I think this is a question that is obvious, but I do want to be sure. If one decides to take the Anesthesiology route during residency after med school, does that also provide them with the training required if they were to become or be an intensivist as well? Also I don't know if this is vice versa if you go into critical care can you go to surgeries and do the job of an anesthesiologist? Thank you guys for replies and taking your time... pain medicine fellowship after EM residency?
I'm about to start my intern year in an academic 4 year EM residency program in July. Given the grim future job outlook for EM (an estimated 9000 EM physicians are predicted to be unemployed by 2030) I've been looking into fellowship options so that I have a backup plan in case I can't land an EM attending job after residency. I've done rotations as a student in typical EM fellowship fields (ultrasound, critical care, etc) but to be honest I just didn't find them that interesting. Pain... Read more

Vein and lymphatic medicine (VLM) fellowship after EM residency?
Incoming 1st year intern into a 4 year EM residency here. Given the doom and gloom surrounding the job market for EM I've been giving some considerable thought into fellowships. Problem is I literally have no interest in the typical EM fellowships (US, critical care, etc.). I recently took a family member to a vein clinic (to treat her varicose veins) and noticed a couple of the physicians there were EM board certified but practicing in the vein clinic. A bunch of them work part time in the... Read more

IM/Anes vs. IM/EM for Career in Academic Critical Care
Hello, I am a third-year medical student. I am a former paramedic with a passion for critical care. I would like to pursue a career in academic critical care, but I am unsure which residency would best prepare me for this. Combined IM/Anes, Combined IM/EM, only IM, or only Anes? I have ruled out straight EM because of the projected job market. Thank you!

What’s New and In the Queue for Academic Medicine
Conclusions Early outcomes reveal that a longitudinal, experiential curriculum can provide students with competencies that may prepare them for leadership roles in advocacy, research, and policymaking. Contact with diverse communities inculcates—in medical students with predispositions toward helping underserved populations—the self-efficacy and skills to positively influence underserved, urban communities.

Global payment contract attitudes and comprehension among internal medicine physicians.
CONCLUSIONS: Four years since first engaging in a global payment contract, a majority of internal medicine physician respondents support this decision. Understanding predictors of physician support for GPC involvement within our care organization may help other health systems to approach organizational change. Health system leaders debating GPC involvement should consider engaging physicians via educational interventions geared toward improving GPC support. PMID: 26618228 [PubMed - in process]
