Adherence to national recommendations for safe methotrexate dispensing in community pharmacies.
CONCLUSIONS: Dutch community pharmacies seem to be adherent to most safe dispensing recommendations. However, inconsistencies exist between team members that emphasize the importance of addressing this issue and discussing recommendations within the team, as there is still room for improvement to ensure safe dispensing. "Wang-Bi tablet, a patented Chinese medicine, maintains the balance of Th1/Th2 in mice with collagen-induced arthritis".
CONCLUSION: These results indicate that WBT may produce a therapeutic effect on CIA through maintaining the balance of Th1/Th2 cells, which could result in a decrease in the autoinflammatory disorder observed in RA. PMID: 32506853 [PubMed - in process]

Methotrexate: a gold standard for treatment of rheumatoid arthritis.
Authors: Shinde CG, Venkatesh MP, Kumar TM, Shivakumar HG Abstract Rheumatoid arthritis (RA) is a painful, debilitating disease characterized by inflammation of the joints, with the proliferation of the synovium and the progressive erosion of cartilage and bone. The treatment of RA is still unsatisfactory, but a number of powerful disease-modifying antirheumatic drugs have become available, such as methotrexate (MTX). Even in the current era of biological targeted therapies, MTX remains the initial preferred antirheumatic drug and is considered to be the gold standard for treatment of RA. The combination of its per...

Potentially severe drug-drug interactions among older people and associations in assisted living facilities in Finland: a cross-sectional study.
CONCLUSIONS: Of the residents in assisted living, 5.9% were exposed to DDDIs associated with the use of a higher number of drugs. Physicians should be trained to find safer alternatives to drugs associated with DDDIs. KEY POINTS   Potentially severe, class D drug-drug interactions (DDDIs) have been defined in the SFINX database as clinically relevant drug interactions that should be avoided.  • Of the residents in assisted living, 5.9% were exposed to DDDIs that were associated with the use of a higher number of drugs.  • The most frequent DDDIs were related to the concomitant use of potassium with amilorid...

Ocular Sarcoidosis.
Abstract Sarcoidosis is one of the leading causes of inflammatory eye disease. Any part of the eye and its adnexal tissues can be involved. Uveitis and optic neuropathy are the main manifestations, which may require systemic treatment. Two groups of patients with sarcoid uveitis can be distinguished: one of either sex and any ethnicity in which ophthalmological findings are various and another group of elderly Caucasian women with mostly chronic posterior uveitis. Clinically isolated uveitis revealing sarcoidosis remains a strictly ocular condition in a large majority of cases. Although it can be a serious conditi...

How Does Pediatric Psoriasis Present?
Discussion Psoriasis is an inflammatory skin disease that is chronic and relapsing with periods of remission. It occurs in genetically susceptible persons and is felt to be triggered by environmental factors including infection (especially Group A, β-hemolytic streptococcus), emotional and physical stress, and skin irritation including friction, rubbing, pressure and scratching. It is common in patients with certain HLA types including HLA-Cw*0602, HLA-Cw6, IL-15 plus others. It occurs in about 1% of the general population with two age-onset peaks: 16-22 years and ~60 years, but can occur at any age. Some studies show...

‘Turbocharged artificial intelligence’ could personalize combination therapy in pediatric leukemia
In this study, patient records were obtained on the dosing of 6-mercaptopurine and methotrexate, as well as on the corresponding  absolute neutrophil count, or levels of a subset of white blood cells called neutrophils that are vital for staving off potentially life-threatening infections. These records showed multiple instances where conventional chemotherapy doses caused deviations from acceptable neutrophil levels. Using the personalized medicine technology, individualized three-dimensional maps were generated to determine the optimal 6-mercaptopurine/methotrexate drug ratios. The team members found that their technolog...

Is Methotrexate Ototoxic? Investigating the Ototoxic Late Effects of Pediatric Cancer Treatment
CONCLUSIONS: The results from this study suggest that children who receive therapies not clinically established as ototoxic (i.e., methotrexate) may still be at a high risk of developing long-term hearing loss as a late effect. Due to the high incidence rate of hearing loss among patients with pediatric cancer, we recommend that audiologists be part of the late-effects care team. This study also demonstrates that patients with pediatric cancer treated with methotrexate should receive routine long-term auditory monitoring as part of their standard of care to detect and manage hearing loss early, minimizing adverse outcomes....
