Left atrial appendage closure in non-valvular atrial fibrillation.
Authors: Häusler KG, Landmesser U Abstract Atrial fibrillation is the most frequent cardiac arrhythmia worldwide, causing approximately 20% of all ischemic strokes. Therefore, oral anticoagulation is recommended in patients with atrial fibrillation with at least a moderate risk of stroke; however, there is a significant proportion of patients who cannot undergo long-term oral anticoagulation. As the left atrial appendage is of major relevance for atrial fibrillation-induced thrombus formation, catheter-based or surgical closure of the left atrial appendage appears to be a promising therapeutic option. Large regi...

Six months follow-up of protected high-risk percutaneous coronary intervention with the microaxial Impella pump: results from the German Impella registry
Conclusions: Patients undergoing protected high-risk percutaneous coronary intervention with Impella support showed an acceptable 180-day clinical outcome regarding major adverse cardiac event and mortality.

Left atrial appendage closure in patients with chronic kidney disease: results from the German multicentre LAARGE registry
ConclusionsDespite an increased cardiovascular risk profile of CKD patients, device implantation was safe, and LAAC was associated with effective stroke prevention across all CKD stages.

Risk of mortality following transcatheter aortic valve replacement for low-flow low-gradient aortic stenosis
ConclusionIn patients with LF-LG AS, history of myocardial infarction, renal dysfunction, tricuspid regurgitation, a low stroke volume index, self-expandable device, and non-femoral approach were associated with increased 1-year mortality after TAVR. The RRS might serve as a helpful tool for risk prediction and patient selection for TAVR in patients with LF-LG AS.

Attitudes toward Anticoagulation for Postoperative Atrial Fibrillation: A Nationwide Survey of VA Providers
ConclusionOur data indicate a wide variability in the management of POAF. This reflects conflicting recommendations in the guidelines, as well as a paucity of prospective treatment trials in this field. Nevertheless, a growing evidence base suggests that this complication carries potentially serious long term morbidity and mortality, and better evidence for its management is needed. This article is protected by copyright.
Nevertheless, a growing evidence base suggests that this complication carries potentially serious long ‐term morbidity and mortality, and better evidence for its management is needed.

The impact of basic atrial rhythm during catheter ablation of atrial fibrillation on clinical outcomes: Lessons from the German Ablation Registry
ConclusionBasic atrial rhythm at the time of AF ablation did not affect long ‐term rhythm outcome. For patients suffering from persistent AF a more favorable acute and long‐term safety profile was observed when ablated in SR.This article is protected by copyright. All rights reserved.

Interventional occlusion of left atrial appendage in patients with atrial fibrillation. Gender ‐related outcomes in the German LAARGE Registry
ConclusionLAARGE documented in this elderly patient population undergoing LAAO implantation a higher rate of severe periprocedural complications in females. At 1-year follow-up similar efficacy and safety outcomes were observed for both genders. This article is protected by copyright. All rights reserved.

Coronary artery bypass grafting versus stent implantation in patients with chronic coronary syndrome and left main disease: insights from a register throughout Germany
ConclusionLeft main stenting is a safe and effective treatment option for CCS-patients suffering from left main coronary artery disease at reasonable economic cost.Graphic abstractCoronary artery bypass grafting versus stent implantation in patients with chronic coronary syndrome and left main disease: insights from a register throughout Germany. All cases with chronic coronary syndrome and left main stenosis treated in 2018 in Germany either with left main stenting or coronary bypass grafting were extracted from a nation-wide database. In-hospital outcomes were compared after logistic regression analysis.

In-hospital outcomes of self-expanding and balloon-expandable transcatheter heart valves in Germany
ConclusionsWe find broadly equivalent outcomes in contemporary TF-TAVR procedures, regardless of the valve type used. Incidence of major complications is very low for both types of valve.

Refined atrial fibrillation screening and cost-effectiveness in the German population
Conclusions In our middle-aged population cohort, we identified 0.2% new AF by single timepoint screening. There was a significant estimated risk of stroke and heart failure in these individuals. Cost-effectiveness for screening may be reached in individuals aged 65 years and older. The simple age cut-off is not improved by using Nt-proBNP as a biomarker to guide a screening programme.

Implication of pulmonary hypertension in patients undergoing MitraClip therapy: results from the German Transcatheter Mitral Valve Interventions (TRAMI) Registry
ConclusionsDespite higher mortality in patients with elevated sPAP, these data suggest the safety, feasibility and benefit of MitraClip therapy even in advanced stages of disease. An early approach might prevent the progress of pulmonary hypertension and improve outcomes.
