Sodium Acetate as a Replacement for Sodium Bicarbonate in Medical Toxicology: a Review.
Abstract Sodium bicarbonate is central to the treatment of many poisonings. When it was placed on the FDA drug shortage list in 2012, alternative treatment strategies to specific poisonings were considered. Many hospital pharmacies, poison centers, and medical toxicologists proposed sodium acetate as an adequate alternative, despite a paucity of data to support its use in medical toxicology. The intention of this review is to educate the clinician on the use of sodium acetate and to advise them on the potential adverse events when given in excess. We conducted a literature search focused on the pharmacology of sod...

Cost-effectiveness of dedicated dietitians for hyperphosphatemia management among hemodialysis patients in Lebanon: results from the Nutrition Education for Management of Osteodystrophy trial.
CONCLUSIONS: DD yielded the greatest effectiveness and decrease in societal costs, but did not affect QALY. Regarding serum phosphorus, DD was likely to be cost-effective compared with EP but had a low cost-effectiveness probability compared with THD. Regarding QALY, DD was not likely to be cost-effective. Assessing the long-term cost-effectiveness of DD, on similar groups, is recommended. PMID: 28657451 [PubMed - as supplied by publisher]

TVA Medical wins CE Mark for EverlinQ 4 endoAVF
TVA Medical said today it won CE Mark approval in the European Union for its EverlinQ 4 endoAVF system designed for creating hemodialysis access using an endovascular approach. The Austin, Texas-based company said the system consists of 2 magnetic catheters which are inserted into an artery and vein in the arm and use a small amount of radiofrequency energy to connect both to create a fistula. The catheters are then removed and the brachial vain is coil-embolized for future dialysis. “The new system with smaller profile catheters and radiopaque visual indicators was extremely easy to use. This new approach streamline...

Improved Education For Kidney Failure Patients May Increase Chances Of Living Donor Transplantation
Patients with kidney failure who have greater transplant knowledge and motivation are ultimately more likely to receive a kidney transplant from a living donor, according to a study appearing in an upcoming issue of the Clinical Journal of the American Society of Nephrology (CJASN). The findings suggest that improving patient education may help reduce disparities in transplantation. A kidney transplant is the best treatment for patients with kidney failure, offering patients a longer and healthier life than dialysis...

Business of dialysis and the role of the medical director
Abstract Since the inception of the Medicare End Stage Renal Disease Program in 1972, the medical director has been an important leader in the dialysis unit. The initial duties of the medical director were focused on quality and safety but were gradually expanded over the decades to include the development and oversight of protocols to manage metabolic bone disease and anemia. As the total cost of ESRD care has escalated, there have been progressive attempts to control costs through additional bundling and the creation of alternative payment schemes. While we await the financial and clinical outcomes of these initiatives, ...

Acute Start—Chronic Needs: Education and Support for Adults who have had Acute Start Dialysis
Abstract For those patients with CKD followed in a pre‐dialysis environment, it is well appreciated that modality education leads to an informed decision regarding type of dialysis selected. However, for those individuals starting dialysis acutely and requiring chronic renal replacement therapy, modality education may be lacking. Because of the acuity of the dialysis start, and the intercurrent illness or event which precipitated and acute start, these patients require a specialized approach to education, and indeed, require a combination of education and support to learn about and choose a renal replacement therapy. The...

Strategies for Setting Medical Nutrition Therapy Priorities for Patients With Stage 3 and 4 Chronic Kidney Disease
The provision of medical nutrition therapy (MNT) for patients with early chronic kidney disease (CKD) has become more common since Medicare began reimbursing for this service in January 2002. This is good news for patients. It has been demonstrated that nutrition care for predialysis patients can improve survival during the first year of dialysis.

Role of telehealth in renal replacement therapy education
Abstract The prevalence of end‐stage renal disease is rising in the United States, which bears high financial and public health burden. The most common modality of renal replacement therapy (RRT) in the United States is in‐center hemodialysis. Many patients report lack of comprehensive and timely education about their treatment options, which may preclude them from participating in home‐based dialysis therapies and kidney transplantation evaluation. While RRT education has traditionally been provided in‐person, the rise of telehealth has afforded new opportunities to improve upon the status quo. For example, techno...

End ‐stage renal disease treatment options education: What matters most to patients and families
Abstract Treatment modality education can offer many important benefits to patients and their families. Evidence suggests such education can increase use of home dialysis, reduce catheter use, decrease 90‐day mortality, and increase transplantation. While these benefits are encouraging, not all patients are offered options education and when they are, it may not be presented in a way that is immediately applicable to them and their lives. Furthermore, little is known regarding specific characteristics (e.g. format such as group or individual or in‐person or online, duration, teaching methods, location, content) of educ...

Home hemodialysis education during postdoctoral training: Challenges and innovations
Abstract Inadequate education in home hemodialysis (HHD) fellowship training might contribute to underutilization of this modality in the United States. Most graduates of nephrology fellowships do not grade themselves as competent in HHD suggesting that fellowship training in HHD is inadequate. An essential component for fellow education is at least one faculty member with expertise in HHD who is passionate about promoting the use of this modality. At a minimum, fellow training should utilize a curriculum that includes both lectures about HHD and outpatient clinical exposure to this modality over a period of at least 6‐1...
