Remission induction comparing infliximab and high-dose intravenous steroid, followed by treat-to-target: a double-blind, randomised, controlled trial in new-onset, treatment-naive, rheumatoid arthritis (the IDEA study)
Conclusions In DMARD-naive early RA patients, initial therapy with MTX+high-dose intravenous steroid resulted in good disease control with little structural damage. MTX+IFX was not statistically superior to MTX+intravenous steroid when combined with a treat-to-target approach.

Long-term Multimodal Therapy (Verapamil Associated with Propolis, Blueberry, Vitamin E and local Diclofenac) on Patients with Peyronie's Disease (Chronic Inflammation of the Tunica Albuginea ). Results of a Controlled Study.
CONCLUSION: Our results showed that a long-term multimodal medical therapy (Verapamil associated with Propolis, Blueberry, Vitamin E and local Diclofenac) is statistically effective in treating PD, whereas, lower therapeutic responses were obtained after 6 months treatment (medium-term treatment). Furthermore, this study strongly supports the recommendation that the best approach for treating PD is multimodal therapy. PMID: 24304332 [PubMed - as supplied by publisher]

Diet of hookworms to tackle a bread allergy

None of them are infected with malaria, but they are in very high numbers. We also have bed bugs, house dust mi...

Treatment to lipid targets in patients with inflammatory joint diseases in a preventive cardio-rheuma clinic
Conclusions There was indication for cardiovascular prevention in a high proportion of IJD patients referred for cardiovascular risk stratification. Treatment to lipid targets was successful in approximately 90% of patients with IJD.

The ideal puncture approach for PCNL: Fluoroscopy, ultrasound or endoscopy?
Bannakij LojanapiwatIndian Journal of Urology 2013 29(3):208-213Percutaneous renal access is a common procedure in urologic practice. The main indications are drainage of an obstructed and hydronephrotic kidney and antegrade renal access prior to percutaneous renal surgeries such as percutaneous nephrolithotomy (PCNL) and percutaneous endopyelotomy (EP). The contraindications for this technique are patients with history of allergy to topical or local anesthesia and patients with coagulopathy. The creation of a percutaneous tract into the renal collecting system is one of the important steps for percutaneous renal access. T...

Ultrasound: a new tool for precisely locating posterior iliac crests to obtain adequate bone marrow trephine biopsy specimen
Ultrasound imaging has evolved as a very useful tool in medical science. However, the technique of ultrasound-guided bone marrow trephine biopsy (BMTB) has not been routinely used by haematologists and oncologists to help locate the posterior iliac crest during the BMTB procedure. We have used the technique of ultrasound-guided BMTB in 20 patients diagnosed with various haematological and non-haematological malignant conditions. We found the technique simple and useful in precisely locating the posterior iliac crest. As a result, it facilitates the extraction of a satisfactory core sample or aspiration biopsy and is more s...

The cellular composition of lymph nodes in the earliest phase of inflammatory arthritis
Conclusions This exploratory study suggests that there is increased immune cell activation within lymph nodes of early arthritis patients as well as in autoantibody-positive individuals at risk of developing RA. This method provides a unique tool to investigate immunological changes in the lymph node compartment in the earliest phases of inflammatory arthritis.

The US7 score is sensitive to change in a large cohort of patients with rheumatoid arthritis over 12 months of therapy
Conclusions The comparable developments of the US7 score with clinical and laboratory data illustrates its potential to reflect therapeutic response. Therefore, the novel US7 score is sensitive to change. Patients who switched from one biologic to another exhibited a significant decline in erosions after 12 months, while the erosions scores in the other groups were stable.

In erosive hand osteoarthritis more inflammatory signs on ultrasound are found than in the rest of hand osteoarthritis
Conclusions Inflammatory signs are more frequent in EOA than in non-EOA, not only in erosive joints but also in non-erosive joints, suggesting an underlying systemic cause for erosive evolution.
