Clinical phenotypes and quality of life to define post-COVID-19 syndrome: a cluster analysis of the multinational, prospective ORCHESTRA cohort
EClinicalMedicine. 2023 Jul 21;62:102107. doi: 10.1016/j.eclinm.2023.102107. eCollection 2023 Aug.ABSTRACTBACKGROUND: Lack of specific definitions of clinical characteristics, disease severity, and risk and preventive factors of post-COVID-19 syndrome (PCS) severely impacts research and discovery of new preventive and therapeutics drugs.METHODS: This prospective multicenter cohort study was conducted from February 2020 to June 2022 in 5 countries, enrolling SARS-CoV-2 out- and in-patients followed at 3-, 6-, and 12-month from diagnosis, with assessment of clinical and biochemical features, antibody (Ab) response, Variant o...

Duchenne muscular dystrophy: Current treatment and emerging exon skipping and gene therapy approach
Eur J Pharmacol. 2023 Mar 22:175675. doi: 10.1016/j.ejphar.2023.175675. Online ahead of print.ABSTRACTDuchenne muscular dystrophy (DMD) is an X-linked recessive neuromuscular disorder that causes debilitating muscle weakness and atrophy due to a loss of the dystrophin protein. Patients with DMD are commonly diagnosed at about 3-5 years of age and progressively decline until complications of the disease often result in death at about 20 years of age. While there is no current cure for DMD, several treatment options focus on improving the quality of life and slowing progression of symptoms associated with the disease. The cu...

Use of digital measurement of medication adherence and lung function to guide the management of uncontrolled asthma (INCA Sun): a multicentre, single-blinded, randomised clinical trial
Lancet Respir Med. 2023 Mar 21:S2213-2600(22)00534-3. doi: 10.1016/S2213-2600(22)00534-3. Online ahead of print.ABSTRACTBACKGROUND: The clinical value of using digital tools to assess adherence and lung function in uncontrolled asthma is not known. We aimed to compare treatment decisions guided by digitally acquired data on adherence, inhaler technique, and peak flow with existing methods.METHODS: A 32-week prospective, multicentre, single-blinded, parallel, randomly controlled trial was done in ten severe asthma clinics across Ireland, Northern Ireland, and England. Participants were 18 years or older, had uncontrolled as...

Safety of SABA Monotherapy in Asthma Management: a Systematic Review and Meta-analysis
CONCLUSIONS: Meta-analysis of data from RCTs showed that deaths were rare with SABA reliever monotherapy, and rates of SAEs and DAEs were comparable between SABA reliever and ICS treatment groups. When used appropriately within prescribed limits as reliever therapy, SABA does not contribute to excess rates of mortality, SAEs, or DAEs.PMID:36348141 | DOI:10.1007/s12325-022-02356-2

Symptom Duration and Diabetic Control Influence Success of Steroid Injection in Trigger Finger
CONCLUSIONS: Patients with a co-existing diagnosis of diabetes and an A1c greater than 6.5, ipsilateral concomitant hand disease, or presence of symptoms for greater than 2.5 months should be counseled regarding higher risk of failure of local corticosteroid injection.PMID:35671444 | DOI:10.1097/PRS.0000000000009320

Clinical outcomes after IL-6 blockade in patients with COVID-19 and HIV: a case series
CONCLUSIONS: In this case series of hospitalized PWH with COVID-19 and given IL-6ri prior to the common use of corticosteroids, there are reports of secondary or co-infection in severely ill patients. Comprehensive studies in PWH, particularly with CD4 counts < 200 cells, are warranted to assess infectious and other outcomes after IL-6ri use, particularly in the context of co-administered corticosteroids.PMID:35148782 | DOI:10.1186/s12981-022-00430-x

COVID-19 and Cytokine Storm Syndrome: Are there lessons from Macrophage Activation Syndrome?
Transl Res. 2021 Mar 5:S1931-5244(21)00052-9. doi: 10.1016/j.trsl.2021.03.002. Online ahead of print.ABSTRACTAlthough interest in "cytokine storms" has surged over the past decade, it was massively amplified in 2020 when it was suggested that a subset of patients with COVID-19 developed a form of cytokine storm. The concept of cytokine storm syndromes (CSS) encompasses diverse conditions or circumstances that coalesce around potentially lethal hyperinflammation with hemodynamic compromise and multiple organ dysfunction syndrome. Macrophage activation syndrome (MAS) is a prototypic form of CSS that develops in the context o...
