Goal-directed therapy with bolus albumin 5% is not superior to bolus ringer acetate in maintaining systemic and mesenteric oxygen delivery in major upper abdominal surgery: A randomised controlled trial
CONCLUSION Bolus human albumin 5% was not superior to bolus ringer acetate in maintaining systemic or mesenteric oxygen delivery in elective major upper gastrointestinal cancer surgery, despite the administration of larger volumes of trial fluid in the ringer acetate group. No significant difference was seen in fluid-related complications or LOS. TRIAL REGISTRATION https://eudract.ema.europa.eu/ Identifier: 2013-002217-36.

From the pages of AllergyWatch ®
For this issue of From the Pages of Allergy Watch, there are 3 reviews dealing with various aspects of drug allergy, the focus of this month ’s Annals. The first review is from a study published in the Annals last year, which examines the safety of direct provocation challenges for patients with a diagnosis of penicillin allergy. The next review discusses the experience of a Danish allergy clinic on testing and challenging patients wit h a diagnosis of local anesthetic allergy. The last one is a review of data from patients reporting anaphylaxis after omalizumab.

Markers of Pulmonary Oxygen Toxicity in Hyperbaric Oxygen Therapy Using Exhaled Breath Analysis
Conclusion In this study, after daily HBOT for 6 days, we identified 11 VOCs which could be divided into two PCs. PC1 (inflammation) showed no significant changes over time, PC2 (methyl alkanes) was significantly reduced after daily exposure for 5 days, to approximately 60% after 2 days’ rest. This suggests that daily HBOT does not induce cumulative damage to the pulmonary system of healthy volunteers, but triggers an adaptive response. Additional studies including more treatment sessions, and perhaps additional systemic parameters on radical oxygen scavengers and inflammatory markers, are necessary to substantiate...

ALCAM Mediates DC Migration Through Afferent Lymphatics and Promotes Allospecific Immune Reactions
In conclusion, our findings identify ALCAM as a novel therapeutic target for preventing corneal allograft rejection in high-risk patient populations. Moreover, given the documented contribution of ALCAM to (lymph)angiogenesis, DC migration, and T cell activation, ALCAM blockade might represent a therapeutic strategy for treating other immune-mediated inflammatory conditions, such as solid organ transplant rejection, allergy or autoimmune diseases. Experimental Procedures Cloning, Expression, and Purification of I/F8-Fc and KSF-Fc For cloning of I/F8-Fc and KSF-Fc the respective scFv sequence was amplified from previousl...

Low Incidence of Biphasic Allergic Reactions in Patients Admitted to Intensive Care after Anaphylaxis
Conclusions The authors observed a low incidence of biphasic reactions in patients admitted to an intensive care unit after anaphylaxis, at a rate equivalent to that reported in other patient groups.

Deep neuromuscular blockade and surgical conditions during laparoscopic ventral hernia repair: A randomised, blinded study
CONCLUSION Deep compared with no NMB did not change the rating score of the surgical view immediately after introduction of trocars during laparoscopic ventral hernia repair, but the surgical condition were improved during suturing of the hernia. TRIAL REGISTRATION ClinicalTrials.gov, NCT02247466.

Risk of Immediate-Type Allergy to Local Anesthetics Is Overestimated—Results from 5 Years of Provocation Testing in a Danish Allergy Clinic
ConclusionsNone of the 164 patients with suspected immediate-type allergy to LAs reacted on provocation. Thus, no patients have been diagnosed with an immediate allergy to LAs in our regional allergy clinic in the 5-year period studied, and allergy to LAs must be considered very rare. Alternative mechanisms should be considered, but if symptoms are consistent with allergy, other potential allergens should be investigated.

The Importance of Prolonged Provocation in Drug Allergy — Results From a Danish Allergy Clinic
Conclusions Only 11.0% of the provocations were positive. Importantly, only 1 of 5 patients tested positive on the first dose, indicating that prolonged exposure should always be considered when drug provocation is included in allergy investigations. Most provocations were with penicillins, reflecting the pattern of antibiotic use in Denmark, which differs from that in other countries, especially outside Northern Europe.

Oxytocin use during Caesarean sections in Denmark – are we getting the dose right?
ConclusionToo high oxytocin doses are still used in Denmark leading to dose‐related side effects mimicking allergic reactions. Coordination between obstetricians and anaesthesiologists on producing common updated guidelines on the administration of oxytocin and dissemination of this information to obstetric and anaesthetic departments in Denmark is needed.
