A Left-to-Right Shunt After Transfemoral TAVR Using Edwards SAPIEN 3.
Authors: Steffen J, Köhler A, Schwarz F, Sadoni S, Hagl C, Massberg S, Greif M Abstract Transcatheter aortic valve replacement (TAVR) is used for the treatment of aortic stenosis (AS). Besides major bleeding, conduction blocks, stroke or atrial fibrillation, complications include cardiac perforation with possible left-to-right-shunts. Herein is reported the sixth case of a left-to-right shunt in an 87-year-old man who underwent TAVR using a 29 mm Edwards SAPIEN S3 prosthesis to treat AS. Soon after the procedure, a small channel evolving from the right coronary cusp could be detected on echocardiography. The patie...

Echocardiographic assessment of long-term hemodynamic characteristics of mechanical mitral valve prostheses with different mitral valvular diseases
This study is to compare long-term echocardiographic results of mechanical mitral valve prostheses between MS and MI patients. From January 2003 to January 2009, a total of 199 consecutive patients were recruited in this study. Patients were classified as group MS (n  = 123) and MI (n = 76) according to the manifestation of mitral valvular disease. The mean age for patients was 50.1 ± 10.5 years and follow-up time was 7.2 ± 2.0 years. The MS after operation were more likely to experience atrial fibrillation (p = 0.002). The New York Heart Association (NYHA) class in MI showed a greater improvement (...

End ‐stage renal disease and severe aortic stenosis: Does valve replacement improve one‐year outcomes?
ConclusionsPatients with ESRD and severe AS have a similar and higher survival with TAVR or SAVR when compared to BAV at 1‐year. These results may influence patient care decisions favoring valve replacement in AS patients with ESRD. © 2016 Wiley Periodicals, Inc.

Abstract 155: Prevalence and Clinical Correlates of Left Ventricular Hypertrophy by Sokolow-Lyon and Cornell Electrocardiogram Voltage Criteria in Transcatheter Aortic Valve Replacement Patients Session Title: Poster Session II
Conclusion: The presence of LVH by Sokolow-Lyon and Cornell ECG voltage criteria poorly correlates with the presence of LVH and critical aortic stenosis in TAVR patients. Women are more likely to have voltage criteria for LVH. Therefore, ECG may not be a suitable method of screening patients with severe aortic stenosis for LVH, especially in men.

Analysis shows increased risk of early stroke with new-onset atrial fibrillation post-TAVR
(Society for Cardiovascular Angiography and Interventions) Results from 'Effect of bivalirudin versus unfractionated heparin in patients with baseline or new-onset atrial fibrillation in transcatheter aortic valve replacement: From the BRAVO-3 randomized trial' were presented today as a late-breaking clinical trial at the Society for Cardiovascular Angiography and Interventions (SCAI) 2017 Scientific Sessions in New Orleans.

Comparison of dual antiplatelet therapy versus oral anticoagulation following transcatheter aortic valve replacement: A retrospective single-center registry analysis.
CONCLUSIONS: OAC after TAVR seems to reduce the risk of clinical valve thrombosis without a statistically significant increase in bleeding complications. PMID: 28497845 [PubMed - as supplied by publisher]

Outcomes for Low-Risk Surgical Aortic Valve Replacement: A Benchmark for Aortic Valve Technology
Conclusions Low-risk patients undergoing surgical AVR in the current era have excellent results. The most common complications were atrial fibrillation and bleeding. These real-world results should provide additional context for upcoming transcatheter clinical trial data.

Transcatheter Versus Surgical Aortic Valve Replacement for Aortic Stenosis in Low-Intermediate Risk Patients: A Meta-Analysis
Conclusions While there was no difference in 30-day and late mortality, the rate of complications differed between TAVR and SAVR in the low-intermediate surgical risk population. Teaser In this meta-analysis, there was no difference in 30-day and median 1.5-year mortality outcomes between TAVR and SAVR. However, there was a higher incidence of aortic insufficiency, vascular complications and permanent pacemaker implantation after TAVR compared to SAVR and a higher incidence of atrial fibrillation, acute kidney injury, major bleeding and cardiogenic shock after SAVR compared to TAVR.

Stentless Bioprostheses for Aortic Valve Replacement in Octogenarians: The Influence of Coronary Artery Disease
Conclusion Use of the Freestyle stentless valve prosthesis for AVR is feasible also in octogenarians. The existence of coronary artery disease leads to concomitant bypass surgery, but not a higher level of perioperative or long-term mortality.
