Changes in demographics, treatment and outcomes in a  consecutive cohort who underwent transcatheter aortic valve implantation between 2005 and 2020
CONCLUSION: Over our 15 years' experience, patient age remained unchanged but the patient risk profile became more favourable. Simplification of the TAVI procedure occurred in parallel with major improvement in outcomes and survival. Bleeding/vascular complications and the need for pacemaker implantation remain the Achilles' heel of TAVI.PMID:35212972 | DOI:10.1007/s12471-022-01662-2

Thoracic Aortic < sup > 18 < /sup > F-Sodium Fluoride Activity and Ischemic Stroke in Patients With Established Cardiovascular Disease
CONCLUSIONS: In patients with established cardiovascular disease, thoracic aortic 18F-sodium fluoride activity is associated with the progression of atherosclerosis and future ischemic stroke. Arterial 18F-sodium fluoride activity identifies localized areas of atherosclerotic disease activity that are directly linked to disease progression and downstream regional clinical atherothrombotic events. Transcatheter Aortic Valve Implantation in Low and Intermediate Surgical Risk Patients: a Critical Appraisal of Seminal Studies
AbstractPurposeTranscatheter aortic valve implantation (TAVI) has grown in popularity as an alternative to surgical aortic valve replacement (SAVR) for the management of aortic stenosis. In this review, we perform a critical appraisal of the key studies comparing TAVI and SAVR in aortic stenosis patients with intermediate and low risk of operative mortality.Recent findingsEarly data from TAVI vs. SAVR trials in low and intermediate surgical risk patients suggest equivalent outcomes at 2  years, with some hints of superiority for TAVI over SAVR in the low-risk group. Initial mid-term data are a bit less favorable for TAVI ...

The role of TAVR in patients with heart failure: do we have the responses to all questions?
AbstractSevere aortic stenosis (AS) is the most prevalent valvular heart disease in developed countries. Heart failure (HF) is a frequent comorbidity of this condition and represents a diagnostic and therapeutic challenge. The spectrum of both conditions has become progressively wider in the last decade; HF has been divided in three groups according to left ventricular ejection fraction (LVEF) and severe AS has been reclassified into four groups according to aortic valve (AV) gradient, AV flow measured by LV stroke index, and LVEF. Although all four AS types may be found in patients with signs and symptoms of HF, low-flow ...

Feasibility and safety of transradial balloon aortic valvuloplasty in patients with severe aortic stenosis
AbstractBalloon aortic valvuloplasty (BAV) was developed as a technique to treat aortic stenosis (AS) and is associated with significant improvements in aortic valve area and trans-aortic valve gradient in the early and immediate periods after the procedure. BAV is commonly performed using a trans-femoral retrograde approach; however, trans-femoral access is associated with frequent access-site bleeding. Among 146 patients with symptomatic severe AS who were treated with BAV in our institution, 123 patients received BAV treatment via a trans-radial approach using a 7-Fr Glidesheath. The balloon size was 16 –20 mm for al...

Meta-Analysis of 5-Year Risk of All-Cause Death or Stroke After Transcatheter Aortic Valve Implantation Versus Surgical Aortic Valve Replacement
Transcatheter aortic valve implantation (TAVI) is an established alternative to surgical aortic valve replacement (SAVR) for patients with severe symptomatic aortic stenosis.1,2 Its indication has expanded from high-risk to include a low surgical risk younger population. Although the short-term results of TAVI and SAVR in patients with intermediate or high surgical risk are similar,1,2 outcomes beyond short-term periods remain unclear. Recently, the results of 5-year follow-up of the Surgical Replacement and Transcatheter Aortic Valve Implantation (SURTAVI) trial has been reported, which showed a similar rate of the compos...

Impact of the recovery of left ventricular ejection fraction after TAVI on mortality in patients with aortic stenosis
CONCLUSION: Improvement of LVEF after TAVI, which reflected the marked LV reverse remodeling, has an impact on the prediction of the survival in patients with AS, and this is more prominent in patients with low SVI.PMID:34881698 | DOI:10.5543/tkda.2021.66495
